摘要
目的探讨改良鼻烟窝穿支皮瓣修复手背部大面积皮肤缺损缺损的疗效。方法2014年9月-2017年12月我院收治12例手背部皮肤缺损患者。男性9例,女性3例,年龄21~48岁,平均年龄35岁。致伤原因:机器挤压伤8例,车祸伤4例。皮肤缺损均伴有肌腱、骨质的损伤和外露。一期先行清创,修复损伤骨质、肌腱,二期行皮瓣修复创面,供区行植皮修复。结果术后患者皮瓣均成活,其中2例皮瓣边缘坏死,给予换药后,伤口愈合良好。供区植皮愈合良好,切口均一期愈合。12例均获随访8~14个月,平均10个月。皮瓣外观良好,手部功能良好。结论改良鼻烟窝穿支皮瓣扩大了皮瓣切取面积,手术操作简单,是修复手背部大面积皮肤缺损的良好方法。
Objective To investigate the therapeutic effect of modified snuff -box-flap on repair of large area skin defects on hand. Methods A total of 12 patients with large area skin defects on hand admitted to our hospital from September 2014 to December 2017,including 9 males and 3 females with age ranged from 21 to 48 (average age of 35) years were included in our study.Eight cases injured by machine crushing and 4 cases by road traffic accident.Skin defects were accompanied by damage and exposure of tendons and bone.The first step was to do a debridement,repair the damaged bone and tendon,and then the wound was repaired by flap,and the donor site was repaired by skin graft.Results All flaps survived after operation,marginal necrosis of the skin flap occurred in 2 cases,and it recovered well after dressing.First intention healing occurred in all the wounds of skin graft in donor sites. All cases were followed-up for 8-14 months,with an average of 10 months.The skin flap showed good appearance and well function. Conclusion The modified snuff -box-flap can enlarge the skin flap area,and the operation procedure is simple,which is a good method to repair the large area skin defect on the back of hand.
