摘要
目的:探讨比较不同手术入路椎体成形术治疗老年性骨质疏松压缩性骨折的临床效果。方法:选择35例老年性骨质疏松压缩性骨折患者作为对象,均行经皮椎弓根椎体成形术(PVP),根据不同手术入路,分成两组,设为单侧组(n=15)与双侧组(n=20),对比观察临床疗效。结果:单侧组手术时间比双侧组短,骨水泥量高于双侧组,P<0.05,而组间椎体高度重建率、后凸Cobb角改善无明显差异,P>0.05;单侧组骨水泥渗漏、邻近椎体骨折发生率高于双侧组,P<0.05;术前与术后,两组患者的VAS评分及ODI差异不大,P>0.05。结论:不同手术入路椎体成形术治疗老年性骨质疏松压缩性骨折,均可取得满意效果,相比而言,双侧入路可减少骨水泥渗漏与邻近椎体骨折现象。
Objective:To compare the clinical effects of vertebroplasty with different surgical approaches in the treatment of senile osteoporotic compression fractures.Methods:35 cases of senile osteoporotic compression fracture were selected as the subjects.All patients underwent percutaneous pedicle vertebroplasty(PVP).According to different surgical approache s,they were divided into two groups,one-sided group(n=15)and two-sided group(n=20).Results:The operation time of unilateral group was shorter than that of bilateral group,and the amount of bone cement was higher than that of bilateral group(P<0.05),but there was no significant difference in the rate of vertebral height reconstruction and the improvement of kyphosis Cobb angle between groups(P>0.05);the incidence of cement leakage and adjacent vertebral fracture in unilateral group was higher than that in bilateral group(P<0.05);the VAS score and ODI of the two groups had no significant difference before and after operation(P>0.05).Conclusion:Vertebroplasty through different surgical approaches can achieve satisfactory results in the treatment of senile osteoporotic compression fractures.In contrast,bilateral approaches can reduce cement leakage and adjacent vertebral fractures.
