摘要
目的探讨超声应变力弹性成像与声诺维超声造影对乳腺影像报告和数据系统分类标准(BIRADS)4类结节的鉴别诊断价值。方法纳入2017年1月~2018年6月间收治的82例乳腺BI-RADS4类结节患者作为研究对象,所有患者均于术前行超声应变力弹性成像和声诺维超声造影。以病理诊断为金标准,观察超声应变力弹性成像、声诺维超声造影单独及联合诊断结果,计算诊断效能(准确度、敏感度、特异度、阳性及阴性预测值),绘制ROC曲线对比不同诊断方法的效能。结果超声应变力弹性成像1~3分者37例、4~5分者45例,声诺维超声造影0分者42例、1分者40例;超声应变力弹性成像、声诺维超声造影单独及联合检查诊断BI-RADS4类乳腺良恶性结节ROC曲线下面积分别为0.868、0.816、0.925。结论超声应变力弹性成像联合声诺维超声造影鉴别诊断BI-RADS4类乳腺结节效能良好,且优于两种影像学单独检测。
Objective To investigate the feasibility of categorizing with ultrasound strain elastography and SonoVue ultrasound contrast the breast imaging reporting and data system(BI-RADS)type 4 nodules. Methods 82 cases of patients with breast BI-RADS type 4 nodules admitted between January 2017 and June 2018 were enrolled in the study, and all patients received ultrasound strain elastography and SonoVue ultrasound contrast enhanced imaging before surgery. The pathological diagnosis was taken as the gold standard for the comparison of ultrasonic strain elastography, SonoVue ultrasound contrast enhanced images and combined diagnosis. The diagnostic efficiency(accuracy, sensitivity, specificity, positive and negative predictive value)was calculated, and the ROC curves were drawn to compare the efficiency of different diagnostic methods. Results There were 37 cases of 1 to 3 points and 45 cases of 4 to 5 points using ultrasound elastography, and there were 42 cases of 0 points and 40 cases of 1 points using ultrasound contrast enhanced images. The area under the ROC curve of ultrasound elastography, ultrasound contrast enhanced images and combined diagnosis for benign and malignant BI-RADS type 4 breast nodules were 0.868, 0.816, and 0.925 respectively. Conclusions Ultrasound strain elastography combined with SonoVue ultrasound contrast enhances imaging have good efficiency in categorizing BI-RADS type 4 breast nodules, and is superior to either one of the imaging methods.
