摘要
目的:探讨基于孕早期孕妇的睡眠质量对妊娠期糖尿病的影响。方法:选取2015年2月至2018年5月龙岩人民医院妇产科孕早期妇女600例作为研究对象,分析其孕前期睡眠状态等资料,以匹兹堡睡眠质量指数(PSQI)≥7分作为睡眠障碍的定义,将患者分为睡眠障碍组245例和睡眠正常组355例。并在孕7~8个月进行糖耐量筛查试验(OGTT),并根据妊娠期糖尿病诊断标准进行判断,分析孕早期孕妇睡眠质量对妊娠期糖尿病发病率的影响。结果:睡眠障碍组和睡眠正常组患者的生化检验指标比较,差异有统计学意义(P<0.05)。睡眠障碍组患者发生妊娠期糖尿病的发病率为44.90%(110/245),睡眠正常组的发病率为22.54%(80/355),差异有统计学意义(P<0.05)。单因素分析可见,年龄、学历、孕前BMI值、工作状况属于影响孕早期孕妇睡眠质量的影响因素(P<0.05)。建立多因素非条件logistic回归模型,孕早期睡眠障碍属于妊娠期糖尿病发生的独立危险因素(P<0.05)。结论:孕早期孕妇睡眠障碍与妊娠期糖尿病的发生率有密切关联,值得在临床进一步探讨。
Objective:To explore the influence of sleep quality on gestational diabetes mellitus based on early pregnancy.Methods:A total of 600 women in the Department of gynecology and obstetrics in Longyan People′s Hospital from February 2015 to May 2018 were selected as the study subjects.The data of their sleep status in the early pregnancy were analyzed.The definition of sleep disorder was PSQI(>7).The patients were divided into sleep disorder group(245 cases)and normal sleep group(355 cases).The effect of sleep quality of early pregnant women on the incidence of gestational diabetes mellitus was analyzed by OGTT and the diagnostic criteria of gestational diabetes mellitus.Results:There were significant differences in biochemical indexes between sleep disorder group and normal sleep group(P<0.05).The incidence of gestational diabetes mellitus was 44.90%(110/245)in the sleep disorder group and 22.54%(80/355)in the normal sleep group,with significant difference(P<0.05).Univariate analysis showed that age,educational background,pre-pregnancy BMI and working status were the influencing factors of sleep quality in early pregnant women(P<0.05).Establishment of multivariate unconditional logistic regression model showed that sleep disorder in early pregnancy was an independent risk factor for gestational diabetes mellitus(P<0.05).Conclusion:Sleep disorders in early pregnant women are closely related to the incidence of gestational diabetes mellitus,which deserves further discussion in clinical practice.
