摘要
目的:探讨二维超声联合弹性成像能否提高乳腺占位性病变的诊断准确性。方法:纳入在我院行超声诊断并经病理确诊的患者165例,病灶共200个,分别使用二维超声、弹性成像及两者联合的方式对乳腺占位性病变做出相应诊断,以病理结果作为金标准,判断三种检查方式的准确性。并做ROC曲线。结果:二维超声、弹性成像、两者联合诊断准确性分别为75.0%、74.5%、84.0%,敏感度分别为69.44%、70.83%、72.22%,特异度分别为78.13%、76.56%、90.62%,阳性预测值分别为64.10%、62.96%、81.25%,阴性预测值分别为81.97%、82.35%、85.29%。三者曲线下面积分别为0.883、0.833、0.924。可见两者联合检查在诊断准确性、敏感度、特异度均优于二维超声及弹性成像,后两者在诊断准确性方面无明显差异。结论:二维超声联合弹性成像可提高乳腺占位性病变诊断准确性,可以在临床运用中大力推广。
Objective:To evaluate the accuracy of two-dimensional ultrasound combined with elastography in the diagnosis of breast space occupying lesions.Methods:165 patients,200 nodules of ultrasonic diagnosis and pathological diagnosis were included in our hospital.The diagnosis of breast space occupying lesions was made by two-dimensional ultrasound,elastography and the combination of the two. The accuracy of the three methods was judged by the pathological results as the gold standard.And do the ROC curve.Results:The diagnostic accuracy of two dimensional ultrasound and elastography was 75.0%,74.5%,84.0%,respectively, and the sensitivity was 69.44%,70.83%,72.22% respectively,the specificity was 78.13%,76.56%,90.62% respectively, and the positive predictive values were 64.10%,62.96%,81.25%,respectively,81.97%,82.35%,85.29% and negative predictive values respectively.The area under the curve of the three is 0.883,0.833,0.924.The diagnostic accuracy,sensitivity and specificity of the two combined tests were superior to two-dimensional ultrasound and elastography, and there was no significant difference in diagnostic accuracy between the two.Conclusion:Two dimensional ultrasound combined with elastography can improve the diagnostic accuracy of breast space occupying lesions,and it can be widely applied in clinical practice.
