胎儿肺支气管铸型及三维可视化结构的研究 被引量:5

摘要 目的制作胎儿肺支气管的铸型标本,运用三维重建技术探究胎儿肺支气管的解剖结构,为临床提供形态学资料。方法对1例34周胎儿肺支气管进行插管铸型,通过128层螺旋CT扫描铸型标本,得到Dicom数据并导入Syngo.via Client工作站,以最大密度投影(MIP)的方式重建和导入Mimics19.0软件进行三维重建。结果制作好的支气管树铸型管道充盈饱满,粗细分明,能够清晰显示出胎儿肺支气管的分支形态、管径粗细以及走行分布。MIP重建的图像能够清晰地显示3~5级分支,伪影较少。在Mimics上进行三维重建的胎儿肺支气管形态逼真,可通过其阈值大小来调节各分支末端的疏密情况,并可从多层面、多方面、多角度进行观察,空间立体感较强。结论通过运用血管铸型技术、MIP重建技术和Mimic重建技术,可以清晰地了解胎儿肺支气管走行及分布情况,为临床医学教学和虚拟手术及宫内介入手术提供胎儿肺支气管形态学资料。 Objective The cast specimens of fetal lung and bronchus are made to explore the anatomical structure by using three-dimensional reconstruction technology and provide morphological data for clinical practice.Methods One case of 34 weeks fetal lung and bronchus was cast by intubation.The cast specimens were scanned by 128-slice spiral CT.Dicom data were obtained and imported into Syngo.via Client workstation to reconstruct the lung and bronchus by maximum density projection(MIP)and to import Mimics 19.0 software for three-dimensional reconstruction.Results The cast tube of the bronchial tree is full,thick and detailed,which can clearly show the shape,diameter and distribution of the branches of the fetal lung and bronchus;the MIP reconstructed image can clearly show the branches of grade 3 to 5 with fewer artifacts;the shape of the fetal lung and bronchus reconstructed on mimics is realistic,and the sparsity of the ends of each branch can be adjusted by its threshold size,which can be observed from multiple multi-level,multi-faceted,multi-angle,with stong sense of three-dimensional space.Conclusion The fetal lung and bronchus distribution can be understand by vascular casting technology,MIP reconstruction technology and mimic reconstruction technology,which provide the morphological data of fetal lung and bronchus for clinical medicine teaching,virtual surgery and intrauterine interventional surgery.
