摘要
目的:探讨葛根芩连汤配方颗粒联合基础治疗对2型糖尿病(Type 2 Diabetes Mellitus,T2DM)气阴两虚证患者糖代谢指标和胰岛功能的影响。方法:选取唐山市中医医院2017年5月-2018年5月就诊的T2DM气阴两虚型患者160例,随机分为对照组和实验组,每组80人。在服用二甲双胍的基础上,对照组给予葛根芩连汤传统中药汤剂,实验组给予葛根芩连汤配方颗粒治疗,两个月为一个疗程。检测治疗前和疗程结束后实验组和对照组空腹血糖(Glucose,GLU)、餐后2h血糖(Postprandial 2h Blood Glucose,2hGLU)、糖化红蛋白(HbA1c)、胰岛素分泌指数(Insulin Secretion Index,HOMA-IS)、空腹胰岛素(Fasting Insulin,FINS)、胰岛素抵抗指数(Insulin Resistance Index,HOMA-IR)、血清一氧化氮(Nitric Oxide,NO)、内皮素-1(Endothelin-1,ET-1)、血清白细胞介素-6(Interleukin-6,IL-6)和C反应蛋白(C-Reactive Protein,CRP)指标的改变。结果:葛根芩连配方颗粒中8种主要有效成分高于传统汤剂,且差异具有显著统计学意义(P<0.05)。实验组和对照组总有效率分别为91.25%、75.00%,差异有统计学意义(P<0.05)。治疗前,实验组和对照组GLU、2hPG、HbA1c、FINS、HOMA-IR、CRP、ET-1、IL-6比较差异无统计学意义。治疗后,两组前述各项指标均降低,且实验组降低水平大于对照组,组间差异有统计学意义(P<0.05)。两组HOMA-IS、NO指标在治疗前无统计学差异(P>0.05),治疗后两组HOMA-IS、NO值升高,且实验组升高水平大于对照组(P<0.05)。结论:葛根芩连配方颗粒比传统汤剂有效成分含量更高,降糖效果显著,同时可以改善胰岛功能。
Objective:To explore efficacy of the Gegen Qinlian granule (葛根芩连汤配方颗粒) combined with basic treatment on glucose metabolism and islet function in patients with type 2 diabetes mellitus with the Qiyin Liangxu syndrome (气阴两虚证). Methold:160 patients with type 2 diabetes treated in TangShan hospital of traditional Chinese medicine from May 2017 to May 2018 were selected and randomly divided into the control group and the experimental group,80 patients in each group.On the basis of metformin treatment,the control group was given the Gegen Qinlian decoction,and the experimental group received concentrated granule of the Gegen Qinlian decoction for two months.Glucose (GLU),postprandial 2 h blood glucose (2 hPG),glycosylated red protein (HbA1c), insulin secretion index (HOMA-IS),fasting insulin (FINS),insulin resistance index (HOMA-IR),serum nitric oxide (NO),endothelin-1 (ET-1),interleukin-6 (IL-6) and C-reactive protein (CRP) were monitored during the study.Results:8 kinds of effective ingredients in Gegen Qinlian concentrated granule were higher,and the difference was statistically significant (P<0.05).The total effective rate in the experimental group and the control group was 91.25% and 75.00%,respectively (P<0.05).Before treatment,GLU,2 h PG,HbA1c,FINS, HOMA-IR,CRP,ET-1 and IL-6 were found no difference in two groups (P>0.05).After treatment,the above indexes in both groups decreased,and there were significant differences between the two groups (P<0.05).There was no statistically significant difference in HOMA-IS,NO before treatment in the two groups (P>0.05);after treatment,both of them were increased and the difference was significant (P<0.05).Conclusion:Compared with traditional decoction,Gegen Qinlian concentrated granule has higher effective component content and clinical efficiency.Additionally,it can effectively improve insulin function.
