摘要
目的:分析清肺方对社区获得性肺炎患者的炎症反应的变化及免疫功能的影响。方法:选取经我院确诊的108例获得性肺炎患者作为研究对象,按照随机数方法随机分成观察组和对照组,每组各54例。对照组给予常规基础治疗,观察组在常规治疗的基础上加用清肺方治疗。比较两组治疗前后免疫调节功能指标(CD3^+、CD4^+、CD4^+/CD8^+)、炎性反应指标[C反应蛋白(C-reaction Protein,CRP)、白介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor Necrosis Factor,TNF-α)],评价两组治疗前后中医证候积分及肺炎严重指数(Pneumonia SeverityIndex,PSI)评分,并记录药物不良反应情况。结果:两组治疗后CD3^+、CD4^+、CD4^+/CD8^+比值均较前上升,前后对比差异有统计学意义(P<0.05),观察组治疗后CD3^+、CD4^+、CD4^+/CD8^+比值高于对照组,差异有统计学意义(P<0.05);观察组总有效率为96.30%,高于对照组的83.33%。两组未见药物不良反应发生。结论:常规治疗方法联合中医清肺方对社区获得性肺炎患者有良好的免疫调节效果,提升患者抗感染免疫力,且安全性好,值得推广。
Objective:To analyze the effects of Qingfei Fang (清肺方) on the changes of inflammatory response and immune function in patients with community acquired pneumonia. Methods: 108 patients with acquired pneumonia diagnosed by our hospital were selected as the study subjects. They were randomly divided into the observation group and the control group according to random number method, 54 cases in each group. The control group received routine basic treatment. The observation group was given Qingfei Fang more. The immunomodulatory function indicators (CD3^+, CD4^+, CD4^+/CD8^+) and inflammatory response indicators (CRP, IL-6, TNF-α) and the TCM syndromes before and after treatment in both groups were compared. TCM syndrome scores and PSI scores were evaluated and adverse drug reactions were recorded. Results: The ratios of CD3^+, CD4^+ and CD4^+/CD8^+ increased significantly in the two groups after treatment. The difference between the two groups was statistically significant (P<0.05). The ratio of CD3^+, CD4^+, CD4^+/CD8^+ in the observation group was higher than that in the control group (P<0.05). The total efficiency in the observation group was 96.30%, which was higher than 83.33% in the control group. No adverse drug reactions occurred in the two groups. Conclusion: Conventional treatment plus Qingfei Fang has good immunomodulatory effects on patients with community acquired pneumonia, enhances anti-infective immunity, and is safe and worth promoting.
