摘要
目的探讨快速康复外科护理对胸腔镜肺段切除术患者术后恢复的影响。方法于2016年1月—2017年1月,选取复旦大学附属华山医院胸外科拟行胸腔镜肺段切除手术的患者126例为研究对象,采用随机数表法将其分为观察组(n=63)和对照组(n=63)。对照组采用传统的胸外科围手术期护理,观察组患者在围手术期给予快速康复外科护理,比较两组患者术后恢复指标、并发症发生率、术后7d的生存质量评分以及出院时患者对护理工作的满意度。结果观察组术后胸腔引流管放置时间、首次排便时间、术后住院天数与并发症的总发生率低于对照组(P<0.05),观察组术后7d生存质量量表的生理、心理、社会关系领域评分及总分均高于对照组(P<0.05),观察组护理工作满意度优于对照组(P<0.05)。结论快速康复外科护理对胸腔镜肺段切除术患者具有较好的意义,能促进术后康复,减少术后并发症的发生,提高术后生活质量,值得临床推广应用。
Objective To explore the effect of fast-track surgery (FTS) nursing on postoperative rehabilitation of patients with thoracoscopic segmentectomy.Methods A total of 126 patients who received thoracoscopic segmentectomy in Huashan Hospital affiliated to Fudan University from January 2016 to January 2017 were enrolled in the study and divided into observation group (n=63) and control group (n=63) by means of random number table.Patients in control group were given the traditional perioperative nursing of thoracic surgery,while patients in observation group were given comprehensive nursing care of FTS during the perioperative period.Postoperative rehabilitation indexes,incidence of complications,the World Health Organization Quality of Life-BREF (WHOQOL-BREF) score at the 7th day after operation and nursing satisfaction at discharge were compared between two groups.Results Postoperative placement time of chest tube,the first defecation time,length of postoperative hospital stay and the total incidence of complications in the observation group were significantly lower than those in control group (P<0.05).The scores of physiological,psychological and social relations fields and total scores of WHOQOL-BREF at the 7th day after operation in observation group were significantly higher than those in control group(P<0.05),and the nursing satisfaction was significantly better than that in control group (P<0.05).Conclusion The application of FTS nursing during the perioperative period of thoracoscopic segmentectomy has great clinical significance.It could significantly promote postoperative rehabilitation,reduce risk of postoperative complications,improve postoperative quality of life and nursing satisfaction,which is worthy of clinical application.
