双源CT双低模式在肺动脉成像中的应用研究 被引量:2

摘要 目的:研究128排双源CT低千伏管电压辐射剂量快速闪光Flash扫描模式结合低剂量造影剂在肺动脉血管成像(CTPA)中的应用价值。方法:回顾性分析2017年6月至2018年6月于深圳市第二人民医院接受CTPA检查的72例疑似肺栓塞患者,按扫描方式分为双低组和常规组,每组36例,造影剂碘帕醇以370 mgI·mL^-1的浓度使用。双低组扫描管电压为100 kV,造影剂用量为35 mL,常规组扫描管电压为120 kV,造影剂用量为60 mL。比较两组扫描图像的肺血管CT值、信噪比(SNR)、对比度噪声比(CNR)、CT容积计量指数(CTDIvol)、剂量长度乘积(DLP)、有效辐射剂量(ED)及图像质量的主观评分。结果:双低组上腔静脉及肺静脉CT值〔(98.81±10.44)Hu;108.08±5.01)Hu〕均低于常规组〔(166.47±18.24)Hu;(144.89±14.16)Hu〕,但肺动脉主干、左肺动脉干及右肺动脉干CT值〔(372.22±14.09)Hu;(359.22±12.67)Hu);(364.58±12.51)Hu〕则高于常规组〔(364.72±13.12)Hu;(352.67±10.67)Hu;(358.58±10.48)Hu〕,差异具有统计学意义(P <0.05);两组的图像质量评分差异无统计学意义(P> 0.05),双低组SNR、CNR、CTDIvol、DLP及ED显著低于常规组,组间比较,差异具有统计学意义(P <0.05)。结论:低管电压联合低剂量对比剂在双源CT快速闪光Flash扫描模式行CTPA检查时可获得满意的图像质量,同时,它可以显著减低受试者的辐射剂量和造影剂的量。 Objective To explore the application value of low tube voltage combined with low dosage of contrast agent in Flash mode dual-source computed tomography pulmonary angiography(CTPA). Methods Retrospective analysis of 72 patients who underwent CTPA examination was performed. According to the scan mode, the patients were divided into double low(DL)group and conventional dose(CD) group with 36 patients in each group. All the patients were subjected to CTPA examination by using contrast agent Iopamidol with the concentration of 370 mgI·mL^-1. In DL group, the tube voltage of scanning was 100 kV and the dosage of contrast agent was 35 mL, while that were 120 kV and 60 mL in CD group, respectively. Then compared the differences of CT values in the major pulmonary vessels, signal and noise ratio(SNR), contrast and noise ratio(CNR), volumetric CT dose index(CTDIvol), dose-length product(DLP), effective radiation dose(ED) and the image quality scores between the two groups.Results The CT values of vena cava and pulmonary vein in DL group(98.81 ± 10.44 Hu;108.08 ± 5.01 Hu) were all lower than those in CD group(166.47 ± 18.24 Hu;144.89 ± 14.16 Hu), while the CT values of pulmonary arteries in DL group(pulmonary trunk:372.22 ± 14.09 Hu;left pulmonary artery: 359.22 ± 12.67 Hu;right pulmonary artery: 364.58 ± 12.51 Hu) were higher than those in CD group(pulmonary trunk: 364.72 ± 13.12 Hu;left pulmonary artery: 352.67 ± 10.67 Hu;right pulmonary artery 358.58 ± 10.48 Hu),with statistically significant differences(P < 0.05). The differences of the image quality scores between the two groups were not statistically significant(t =-0.937, P = 0.352), while the SNR, CNR and CTDIvol, DLP and ED in the DL group were significantly lower than those in CD group(P < 0.05), and the radiation dose was reduced significantly in DL group compared to CD group.Conclusion Low tube voltage combined with low dose contrast agent can obtain satisfactory image quality by CTPA examination on dual-source CT Flash mode, and can significantly red
