摘要
目的探讨研究心率和心率变异性与心力衰竭及预后的相关性。方法回顾性分析2015年6月-2017年8月在该院心内科治疗的90例慢性充血性心力衰竭患者的临床资料,根据住院期间平均静息心率水平分为3组,A组心率50~70次/min,B组71~90次/min,C组超过90次/min,分析不同心率与预后情况;根据心功能分为Ⅱ级组,Ⅲ级组、Ⅳ级组,分析心功能与心率变异性关系。结果所有患者随访1年,A组再住院率(33.33%vs 53.33%、70.00%)、病死率(3.33%vs 16.67%、36.67%)显著低于B组和C组,对比差异有统计学意义(P<0.05);随访终点LVEF显著高于B组和C组,而随访终点LVEDD明显低于B组和C组,对比差异有统计学意义(P<0.05)。心功能Ⅱ级组、Ⅲ级组、Ⅳ级组之间SDNN、SDANN、PNN50、RMSSD相比,组间均差异有统计学意义(P<0.05)。结论心率和心率变异性与心力衰竭预后有密切联系,心率越快、心率变异性越大则心功能损害越明显,预后越差。
Objective To investigate the correlation between heart rate, heart rate variability, and heart failure and prognosis. Methods The clinical data of 90 patients with chronic congestive heart failure treated in our hospital from June 2015 to August 2017 were retrospectively analyzed. According to the average resting heart rate level during hospitalization, they were divided into three groups: group A(50-70 beats/min), group B(71-90 beats/min), and group C(more than 90 beats/min). Different heart rate and prognosis were analyzed. According to cardiac function, it was divided into Ⅱ group,Ⅲgroup, and Ⅳ group. The relationship between cardiac function and heart rate variability was analyzed. Results All patients were followed up for one year, the re-hospitalization rate in group A was 33.33% vs 53.33%, 70.00%, and mortality rate was 3.33% vs 16.67%, 36.67%, which were significantly lower than those in group B and C. The difference was statistically significant(P<0.05). LVEF of group A was significantly higher than that of group B and C at the end point of follow-up.LVEDD was significantly lower than that of group B and C at the end point of follow-up. The difference was statistically significant(P<0.05). There were significant differences in SDANN, PN50 and RMSSD among the cardiac function class II,III and IV groups(P<0.05). Conclusion Heart rate and heart rate variability are closely related to the prognosis of heart failure. The faster the heart rate and the greater the heart rate variability, the more obvious the impairment of cardiac function and the worse the prognosis.
