摘要
目的探究后Pilon骨折的手术治疗。方法回顾分析本院2016年1月至2017年12月期间接诊的60例后Pilon骨折患者,按照手术入路方式的不同,将其分为后内侧入路组,即甲组,30例;前外侧入路组,即乙组,30例,分析比较两组骨愈合时间,以及术后并发症发生率、踝关节功能恢复情况。结果治疗后,两组骨愈合时间比较差异无统计学意义;甲组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);甲组踝关节功能恢复优良率(90.00%)远远高于对照组(63.33%),差异有统计学意义(P<0.05);随访期间,发现甲组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论对后Pilon骨折患者采取后内侧入路手术的临床疗效优于前外侧入路,并发症少,且踝关节功能恢复良好,因此,在实际工作中,应当结合患者具体病情,选取最为合适的手术入路方式。
Objective To explore the surgical treatment of posterior Pilon fracture. Methods A retrospective analysis was made of 60 cases of Pilon fracture treated in our hospital from January 2016 to December 2017. According to the different surgical approaches, they were divided into posteromedial approach group, group A, 30 cases, anterolateral approach group, group B, 30 cases. The healing time of bone, the incidence of complications and the recovery of ankle function were analyzed and compared between the two groups. Results After treatment, there was no significant difference in bone healing time between the two groups;the incidence of complications in group A was lower than that in control group (P<0.05);The excellent and good rate of ankle joint function recovery in group A (90.00%) was much higher than that in control group (63.33%), and the difference was statistically significant (P<0.05);during the follow-up period, it was found that the quality of life score of group A was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The clinical effect of posteromedial approach for posterior Pilon fracture is better than that of anterolateral approach, with fewer complications and good recovery of ankle function. Therefore, in practice, the most suitable surgical approach should be selected according to the specific conditions of patients.
