摘要
目的探究经皮肝胆管镜(PTCS)联合负压鞘钬激光碎石术应用于复杂肝胆管结石的临床疗效。方法选取2016年2月至2018年2月本院收治的76例复杂肝胆管结石患者作为本次研究的对象,采用随机数字表法将患者分为两组,观察组38例行PTCS联合负压鞘钬激光碎石术治疗,对照组38例采用传统的开腹手术治疗。比较两组临床疗效、临床指标以及并发症发生情况。结果观察组治疗有效率97.37%显著高于对照组81.58%,差异具有统计学意义(P<0.05)。观察组手术时间、住院时间明显短于对照组,且结石残留率也显著低于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率5.26%显著低于对照组39.47%,差异有统计学意义(P<0.05)。结论PTCS联合负压鞘钬激光碎石术治疗复杂肝胆管结石能够有效改善疗效,且具有创伤小、术后恢复快、并发症发生率低等优势,值得在临床推广应用。
Objective To explore the clinical effect of percutaneous transhepatic cholangioscopy (PTCS) combined with negative pressure sheath holmium laser lithotripsy in the treatment of complex hepatolithiasis. Methods 76 patients with complex hepatolithiasis admitted to our hospital from February 2016 to February 2018 were selected as the subjects of this study. The patients were divided into two groups by random number table. 38 patients in the observation group were treated by PTCS combined with negative pressure sheath holmium laser lithotripsy, while 38 patients in the control group were treated by traditional open surgery. The clinical efficacy, clinical indicators and complications of the two groups were compared. Results The effective rate of the observation group was 97.37%, which was significantly higher than that of the control group (81.58%, P< 0.05). The operation time and hospitalization time of the observation group were significantly shorter than those of the control group, and the stone residual rate was also significantly lower than that of the control group, the difference was statistically significant (P<0.05). The incidence of complications in the observation group was 5.26%, which was significantly lower than that in the control group (39.47%, P<0.05). Conclusion PTCS combined with negative pressure sheath holmium laser lithotripsy can effectively improve the curative effect of complex hepatolithiasis, and has the advantages of small trauma, rapid recovery and low incidence of complications, which is worthy of clinical application.
