摘要
目的 探讨冠心病患者血浆中miR-210、miR-214及基质细胞衍生因子-1 α(stromal cell-derived factor,SDF-1α)的变化与冠状动脉侧支循环形成的相关性.方法 选择2014年2月至2018年5月在我院诊治的冠心病患者140例,检测所有患者血浆中miR-210、miR-214、SDF-1α的含量,检测患者心功能,判定患者的冠状动脉侧支循环形成状况并进行相关性分析.结果 在140例患者中,冠状动脉侧支循环形成80例(观察组),冠状动脉侧支循环未形成60例(对照组),观察组中Ⅰ级45例,Ⅱ级25例,Ⅲ级10例.不同组别的性别、年龄、体重指数、收缩压、舒张压、TG、LDL等对比未见统计学差异(P>0.05).观察组的miR-210相对表达量高于对照组,miR-214相对表达量低于对照组,对比差异有统计学意义(P<0.05).观察组的血浆SDF-1α含量高于对照组[(2543.95±567.01)ng/ml比(2049.88±87.19)ng/ml,t=12.055,P<0.05].观察组的LVEF值高于对照组,Gensini积分低于对照组,对比差异有统计学意义(P<0.05).Spearman相关分析显示,冠心病患者冠状动脉侧支循环形成与血浆miR-210、miR-214和SDF-1α表达有显著相关性(r=0.544、-0.345、0.416,P<0.05).血浆miR-210、miR-214和SDF-1α对冠状动脉侧支循环形成预测的曲线下面积(AUC)分别为0.731、0.802和0.765,诊断的灵敏度分别为72%、81%和61%,特异度分别为64%、66%和75%.结论 冠心病患者中冠状动脉侧支循环形成比较常见,患者血浆中miR-210、miR-214及SDF-1α水平的变化可预测冠状动脉侧支循环形成.
Objective To investigate the relationship between the changes of plasma miR-210,miR-214 and stromal cell-derived factor-1α(SDF-1α)and coronary collateral circulation in patients with coronary heart diseasee.Methods From February 2014 to May 2018,140 patients with coronary heart disease who were treated in our hospital were selected.The plasma levels of miR-210,miR-214 and SDF-1α were measured in all patients.The heart function were measured and the coronary collateral circulation were judged and given correlation analysis.Results Among the 140 patients,there were 80 patients with coronary collateral circulation(observation group)but 60 patients were not with coronary collateral circulation(control group)and there were 45 patients with grade Ⅰ,25 patients with grade Ⅱ and 10 patients with grade Ⅲ heart function.There were no significant differences in gender,age.body mass index,systolic blood pressure.diastolic blood pressure,TG and LDL levels between the different groups(P>0.05).The relative expression of miR-210 in the observation group were higher than that in the control group and the relative expression of miR-214 were lower than that in the control group(P<0.05).The plasma SDF-1α level in the observation group were higher than that in the control group and the difference was statistically significant[(2543.95±567.01)ng/ml vs.(2049.88±87.19)ng/ml,t=12.055,P<0.05].The LVEF value of the observation group were higher than that of the control group but the Gensini score were lower than that of the control group and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that coronary collateral circulation were significantly correlated with plasma miR-210,miR-214 and SDF-1α expression in patients with coronary heart disease(r=0.544,-0.345.0.416,P<0.05).The area under the curve(AUC)of plasma miR-210,miR-214 and SDF-1α for predicting coronary collateral circulation were 0.731,0.802 and 0.765,respectively.The diagnostic sensitivities were 72%,81% and 61%,respectively
