甲状腺手术中喉返神经肉眼识别及神经监测的临床意义分析 被引量:2

摘要 目的分析甲状腺手术中喉返神经肉眼识别及神经监测的临床意义。方法从该院2017年1月—2018年10月接受的甲状腺手术患者中,抽取82例作为研究对象,随机将其分为对照组与观察组,均41例。对照组采用肉眼识别的方式对患者喉返神经进行识别和保护,观察组采用神经监测手段对患者喉返神经进行识别和保护,观察两组患者喉返神经确认时间、识别和损伤情况。结果观察组喉返神经确认时间(2.11±0.53)min显著短于对照组(4.31±1.42)min;喉返神经识别100.00%率显著高于对照组80.49%,差异有统计学意义(P<0.05)。结论在甲状腺手术中采用神经监测对喉返神经进行识别和保护能够有效缩短术中对喉返神经确认的时间,提高对喉返神经的识别率,降低对喉返神经的损伤,是科学、有效的监测手段,值得在临床中推广。 Objective To analyze the clinical significance of visual resection and nerve monitoring of recurrent laryngeal nerve in thyroid surgery. Methods From the patients who underwent thyroid surgery from January 2017 to October2018, 82 patients were selected as subjects. They were randomly divided into control group and observation group, all of which were 41 cases. In the control group, the recurrent laryngeal nerve was identified and protected by visual recognition. The observation group used neurological monitoring to identify and protect the recurrent laryngeal nerve.The recurrence time, recognition and injury of the recurrent laryngeal nerve were observed. Results The recurrent laryngeal nerve confirmation time(2.11±0.53) min was significantly shorter than that of the control group(4.31±1.42)min. 100.00% rate of recurrent laryngeal nerve recognition was significantly higher than that of the control group(80.49%),the difference was statistically significant(P<0.05). Conclusion The use of nerve monitoring in thyroid surgery to identify and protect the recurrent laryngeal nerve can effectively shorten the time of confirmation of the recurrent laryngeal nerve during surgery, improve the recognition rate of the recurrent laryngeal nerve, and reduce the damage to the recurrent laryngeal nerve. It is scientific and effective. The monitoring method is worthy of promotion in the clinic.
