摘要
目的:探讨阿尔茨海默病早期认知功能障碍与血糖、血脂及炎性因子异常的相关性。方法:选取2016年8月至2018年8月在我院就诊的阿尔茨海默病患者100例为观察组,同时选取健康人100例为对照组,分析2组血糖、血脂及炎性因子水平。结果:观察组高敏C-反应蛋白(hs-CRP)、胰岛素抵抗指数(HOMA-IR)、空腹血糖(FBG)、白细胞计数(WBC)、24h尿蛋白(24h Upro)、甘油三酯(TG)、总胆固醇(TC)均高于对照组;脂蛋白a(LP(a))、高密度脂蛋白胆固醇(HDL-C)、血清白蛋白(ALB)、血清总蛋白(TP)、蒙特利尔认知评价量表(MoCA)评分均低于对照组(P<0.05)。2组低密度脂蛋白胆固醇(LDL-C)水平比较差异无统计学意义。通过Pearson相关分析,MoCA评分与HOMAIR、LDL-C、FBG呈负相关(r=-0.73、-0.58、-0.49)(P<0.05),与HDL-C呈正相关(r=0.39)(P<0.05)。多因素Logistic回归分析显示,hs-CRP、LDL-C、LP(a)及FBG与认知功能障碍存在关联。结论:阿尔茨海默病患者存在血糖、血脂及炎性因子异常;hs-CRP、LDL-C、LP(a)及FBG与阿尔茨海默病早期认知功能障碍的关系密切。
Objective: To investigate the correlation between early cognitive impairment and abnormal blood glucose, blood lipids and inflammatory factors in Alzheimer's disease. Methods: A total of 100 patients with Alzheimer's disease from Aug 2016 to Aug 2018 were enrolled as observation group and 100 healthy people as control group. The levels of blood sugar, blood lipid and inflammatory factors in the two groups were analyzed. Results: The levels of hs-CRP, HOMA-IR, FBG, WBC, 24 h Upro, TG and TC in the observation group were higher than those in the control group, and the levels of LP (a), HDL-C, ALB, TP and the scores of MoCA in the observation group were lower than those in the control group (P<0.05). There was no significant difference in the LDL-C level between the two groups. Pearson correlation analysis showed that MoCA score was negatively correlated with HOMA-IR, LDL-C and FBG (r=-0.73,-0.58,-0.49)(P<0.05), and positively correlated with HDL-C (r=0.39)(P<0.05). Multivariate logistic regression analysis showed that hs-CRP, LDL-C, LP(a) and FBG were associated with cognitive dysfunction. Conclusions: Alzheimer's disease patients have abnormal blood glucose, blood lipids and inflammatory factors.hs-CRP, LDL-C, LP(a) and FBG are closely related to early cognitive dysfunction in Alzheimer's disease.
