采用冠状动脉CTA探究血压对主动脉根部及升主动脉的影响 被引量:7

摘要 目的应用冠状动脉CT血管成像(CCTA)探究血压正常者与高血压病人主动脉根部及升主动脉内径的差异,为早期筛查与诊断高血压病人主动脉根部相关疾病提供数据支持。方法回顾性分析2017年1月—6月因疑诊冠心病行CCTA检查的971例病人。其中既往明确诊断为高血压者694例,男388例,平均年龄(53.7±10.3)岁;女306例,平均年龄(58.9±8.5)岁。血压正常且无高血压病史者277例,男165例,平均年龄(52.8±10.1)岁;女112例,平均年龄(58.8±6.8)岁。选用CCTA中心脏收缩期影像,测量主动脉根部及升主动脉4个层面(即主动脉瓣环层面、瓦氏窦层面、窦管交界层面和升主动脉于肺动脉分叉层面)上的管腔内径。采用独立样本t检验比较不同性别高血压组和血压正常组间各个内径测量值的差异。结果血压正常的男性主动脉瓣环短径和长径、瓦氏窦最大内径、窦管交界处内径均高于女性(均P<0.05),升主动脉内径差异无统计学意义(P>0.05)。男性和女性高血压组的升主动脉内径均高于血压正常组(均P<0.05),而高血压组和血压正常组间主动脉瓣环长径、短径,瓦氏窦最大内径,窦管交界处内径差异均无统计学意义(均P>0.05)。结论高血压可以引起升主动脉的显著扩张。 Objective To compare the difference in inner diameter of aortic root and ascending aorta between normotensive and hypertensive patients measured in coronary computed tomography angiography(CCTA),so as to provide data for screening and diagnosing the aortic root related diseases in patients with hypertension at early stage.Methods A total 971 patients with suspected coronary artery disease who underwent CCTA from January to June 2017 were retrospectively analyzed.Among them,694 cases were diagnosed with hypertension,388 were male,with an average age of 53.7±10.3 years,306 were female,with an average age of 58.9±8.5 years.And 277 cases were normotensive without history of hypertension,165 were male,with an average age of 52.8±10.1 years,112 were female,with an average age of 58.8±6.8 years.CCTA images during cardiac systole were used to measure the lumen diameter at four slices across aortic root and ascending aorta[aortic annulus,Valsalva sinus(SOV),sino-tubular junction(STJ),ascending aorta at the pulmonary artery bifurcate level].In the two gender groups,inner diameters of each slice were compared between hypertension group and normotensive group by dependent t test,respectively.Results In the normotensive group,the inner diameters of aortic annulus(maximum diameter and minimum diameter),SOV,and STJ in the male group were significantly higher than in the female group(P<0.05,respectively).There was no significant difference in the inner diameter of ascending aorta between men and women(P>0.05).The inner diameter of the ascending aorta in the hypertension patients was significantly higher than in the normotensive patients(P<0.05).There were no significant difference in the inner diameter of the aortic annulus(maximum diameter and minimum diameter),SOV,and STJ between normotensive and hypertension groups(P>0.05,respectively),regardless of gender.Conclusion Hypertension could induce significant dilatation of ascending aorta.
