摘要
目的探讨成人非中线髓母细胞瘤的MRI表现,以提高对该肿瘤的认识及诊断能力。方法对12例经手术病理证实的成人非中线髓母细胞瘤的MRI表现进行回顾性分析。结果 10例位于小脑灰质区,2例位于桥小脑角区;非中线髓母细胞瘤在T_1WI呈等或稍低信号,T_2WI呈稍高或等信号,信号多不均匀,所有病例均可见囊变区,呈裂隙状、条状及大小不等片状囊变区,增强后8例肿瘤实质呈明显强化,4例呈中度强化,有4例呈渐进性强化。6例瘤周可见轻度水肿,9例幕上脑室有不同程度积液、扩张。结论成人非中线髓母细胞瘤MRI表现有一定的特征性,结合发病部位及仔细分析征象,可提示该诊断的可能。
Objective To define the role of imaging in identifying and classifying the imaging patterns of diagnosic valuve in medulloblastoma in off-midline of adult. Methods A retrospective study was undertaken comprising 12 adults with histopathological evidence of medulloblastoma of off-midline. Results Totally, 10 were located in cerebellar hemisphere, 3 were located in cerebe1lopontine angle. The lesions were oval in 5 cases and irregular in 7 cases. On MRI, the lesions appeared as low or the same intensity on T 1WI and the same or high singal intensity on T 2WI as the cerebellar hemisphere. The cystic components were seen in all cases, as spot, slit-like or large cystic. The solid part of lesions were enhanced at different extent, with 8 cases of marked enhancement and 4 cases of moderate enhancement. Hydrocephaluscases were found in all cases. Conclusion Specific MRI manifestation of medullblastoma in the off-midline of adult are available. With close connection with location and serious observeration, the possibility of medulloblastoma can be suggested.
