慢性肾脏病患者纠正酸中毒对肾功能保护作用的meta分析 被引量:2

摘要 目的进一步明确慢性肾脏病(CKD)患者纠正酸中毒是否对保护肾功能具有积极意义。方法检索PubMed、Embase、Cochrane Library、中国知网等数据库(时间为建库至2018年11月,语言限制:中、英文),从中找到通过碱化治疗纠正酸中毒的干预方式对非透析的CKD患者肾功能影响的随机对照试验。主要的评估结果为患者肾小球滤过率及血清肌酐水平。结果根据设定条件共检索文献531篇,去重后共336篇,经筛选最终纳入5篇文献(367例患者)。与安慰剂对照组比较,使用碳酸氢盐治疗酸中毒的试验组患者肾小球滤过率高、血肌酐低(95%可信区间0.234~0.854、-0.780~-0.170)。结论通过碳酸氢钠纠正CKD患者酸中毒可提高患者的肾小球滤过率,降低其血清肌酐水平,有益于保护肾功能。 Objective To further clear and definite whether correcting acidosis has the positive significance for protecting the renal function in the patients with chronic kidney disease(CKD). Methods The databases of PubMed,Embase,Cochrane Library and CNKI were retrieved for searching the randomized controlled trials (RCT) on the effects of correcting acidosis intervention measure by alkalization therapy on the renal function in the non-dialysis patients with CKD (the retrieval time from the database establishment to November 2018,language limitation:Chinese and English).The main evaluation results were the glomerular filtration rate(GFR) and serum creatine level. Results A total of 531 articles were retrieved according to the setting conditions,there were 336 articles after excluding the duplicated ones,5 articles(367 patients) were finally included after screening.Compared with the placebo control group,GFR in the patients with acidosis treated by sodium bicarbonate group was high and serum creatine level was lower( 95% CI 0.234 to 0.854,-0.780 to -0.170). Conclusion Correcting acidosis by sodium bicarbonate in the patients with CKD can increase GFR,decrease the level of serum creatine and is conducive to protect the renal function.
