摘要
目的分析老年高血压患者在自愿签约“1+1+1”服务模式前后的管理效果,以此推进基于社区的血压有效控制。方法对社区内110例老年高血压患者实施“1+1+1”签约管理半年后,观察和分析患者在签约管理后生活方式的改善情况,高血压健康知识的知晓率以及血压的控制情况。结果对老年高血压患者实施“1+1+1”签约管理后,患者的血压明显低于管理前,两者差异具有统计学意义(P<0.05);患者血压的控制率从管理前的21.82%上升至88.18%,差别具有统计学意义(P<0.05)。结论实施“1+1+1”签约,能够显著提高患者血压的控制率,改善患者的行为生活方式。
Objective To analyze the management effect of elderly patients with hypertension before and after the voluntary signing of the “1+1+1” service model, so as to promote the effective control of blood pressure based on community. Methods A total of 110 elderly hypertension patients in the community under contract management for six months were observed and analyzed for the improvement of their lifestyle, knowledge of hypertension and control of blood pressure. Results After the “1+1+1” contract management for elderly hypertension patients, the blood pressure of the patients was significantly lower than before the management, and the difference between the two was statistically significant (P<0.05). The patient’s blood pressure control rate increased from 21.82% before management to 88.18%, and the difference was statistically significant (P<0.05). Conclusion “1+1+1” can significantly increase the blood pressure control rate and improve the behavior and lifestyle of patients.
