摘要
目的对比研究非扩髓交锁髓内钉(Unreamed Tibial Nail,UTN)与微创经皮钢板内固定术(Minimally Invasive Percutaneous Plateosteosynthesis,MIPPO)治疗GustiloⅠ型胫骨开放性骨折的临床疗效。方法采用随机数字表法将本院2012年1月至2015年12月收入的90例GustiloⅠ型胫骨开放性骨折患者分为采取UTN治疗的UTN组48例及采取MIPPO治疗的MIPPO组42例,对比两组临床疗效。结果两组术中出血量、C臂照射次数、住院时间差异无统计学意义(P>0.05),UTN组手术时间长于MIPPO组,骨折愈合时间短于MIPPO组,差异有统计学意义(P<0.05);治疗前两组Johner-Wruhs评分无明显统计学差异,治疗后差异亦无统计学意义(P>0.05);UTN组骨折畸形愈合率高于MIPPO组,创口感染发生率低于MIPPO组,差异有统计学意义(P<0.05)。结论在GustiloⅠ型胫骨开放性骨折的治疗中UTN与MIPPO均能够取得理想的疗效,临床可根据患者实际情况合理选用。
Objective To compare and study the clinical effects of unreamedtibial nail(UTN) and minimally invasive percutaneous pla-teosteosynthesis(MIPPO) in treatment of open tibial fracture of Gustilo type Ⅰ. Methods 90 cases with open tibial fracture of Gustilo type Ⅰ admitted to our hospital from January,2012 to December,2015 were divided into UTN group of 48 cases(were treated with UTN)and and MIPPO group of 42 cases(were treated with MIPPO) by the random number table method.The clinical effects of two groups were compared. Results There were no significant differences in intraoperative bleeding volume, C arm irradiation times and hospitalization time between the two groups( P>0.05).The operation time in the UTN group was longer than that in the MIPPO group, and the healing time of fracture was shorter than that in the MIPPO group,and the differencesweresignificant(P<0.05).There was no significant difference in Johner-Wruhs score between the two groups before treatment,and there was no significant difference after treatment(P>0.05).The rate of fracture malunion in UTN group was higher than that in MIPPO group, and the incidence of wound infection was lower than that in MIPPO group,and the differencesweresignificant( P <0.05). Conclusion The UTN and MIPPO could achieve ideal results in treatment of Gustilo type Ⅰ open tibial fracture,which can be reasonably selected according to the actual situation of patients in clinical.
