摘要
阐述了彭万年教授"从脾论治"糖尿病的理论基础与治则,结合临床案例分析其"从脾论治"糖尿病的疗效。彭教授认为糖尿病的形成和发展与脾有着密切的关系,糖尿病往往以阳虚为主,脾肾阳虚是其基本病机,常伴气滞、血瘀、痰浊等。治疗糖尿病当从脾肾出发,温阳为主,兼以通血脉、调枢机、化痰浊;根据临床症状的不同,彭教授擅用健脾益气、补肾温阳的小柴胡汤、四君子汤、四逆散、附子汤等经方,在顾护中气的基础上,注重配合使用活血通络、化痰、利水的方药。
This article mainly summarizes the theoretical basis and therapeutic principles of Professor PENG WanNian for the treatment of diabetes mellitus(DM)based on regulating spleen. Combined with clinical feedback,the effect of "treating diabetes mellitus based on regulating the spleen"was investigated. Professor PENG believes that the pathogenesis and development of DM is closely related to the spleen. The pathogenesis of DM is often dominated by Yang deficiency, and spleen-kidney-yang deficiency is its basic pathogenesis, usually accompanied by Qi stagnation,blood stasis,phlegm turbidity and so on. The treatment of DM should start from the spleen and kidney, and the main aspect is to supplement Yang, assistant with unblocking blood vessels,regulating pivot Qi movement,and resolving phlegm. According to the clinical manifestations,professor PENG prefers to adopting classical formulae such as Xiaochaihu Decoction,Sijunzi Decoction,Sini Powder,and Fuzi Decoction for replenishing spleen and strengthening Qi, tonifying kidney and warming Yang. Based on the protection of middle-energizer Qi,professor PENG pays more attention to use the medicine for activating blood to unblock collaterals,resovling phlegm and inducing diuresis.
