摘要
目的探讨彩色多普勒超声联合胎心监护在诊断胎儿宫内缺氧中的价值。方法选取2012年11月至2017年10月于大同市第三人民医院行彩色多普勒超声检查且住院分娩的孕妇82例,采用回顾性分析的方式,将脐血流测定脐动脉收缩期血流速度峰值/舒张末期血流速度(S/D)值≥3的38例孕妇设为实验组,S/D值<3的44例孕妇设为对照组。比较两组胎儿脐血流情况、胎心监护情况、新生儿Apgar评分及监测结果与胎儿宫内缺氧的关系。结果实验组脐血流中阻力指数、S/D、搏动指数值均高于对照组(P<0.05);实验组胎心监护异常率高于对照组(P<0.05)。实验组Apgar评分正常者占52.63%,异常者占47.37%;对照组Apgar评分正常者占97.73%,异常者占2.27%(P<0.05)。胎儿宫内缺氧发生率:胎心监护异常及S/D值≥3者最高,胎心监护正常及S/D值≥3者次之,胎心监护异常及S/D值<3者较低,胎心监护正常及S/D值<3者最低。结论彩色多普勒超声联合胎心监护对诊断胎儿宫内缺氧有重要的参考价值,值得临床推广应用。
Objective To investigate the value of color Doppler ultrasound combined with fetal heart rate monitoring in diagnosis of fetal intrauterine hypoxia. Methods A total of 82 pregnant women who underwent color Doppler ultrasound examination and delivered during November 2012 to October 2017 in the Third People’s Hospital of Datong were selected, and their clinical date were investigated and analyzed retrospectively. Thirty-eight pregnant women whose blood flow velocity of umbilical artery S/D (the ratio of peak blood flow velocity of umbilical artery to end-diastolic blood flow velocity during systolic phase)≥3 were selected as experiment group, and 44 pregnant women with S/D<3 were selected as control group. The relationship between fetal umbilical cord blood flow, fetal heart rate monitoring, neonatal Apgar score, monitoring results and fetal intrauterine hypoxia was compared between the two groups. Results The values of resistance index, S/D and pulsation index in umbilical blood flow of experiment group were higher than those of control group (P<0.05). The abnormal fetal heart rate of experiment group was significantly higher than that of control group (P<0.05). The Apgar score of 52.63% patients of experiment group was in the normal range, and Apgar score of 47.37% patients was in the abnormal range. the Apgar score of 97.73% patients of control group was in the normal range and Apgar score of 2.27% patients was in the abnormal range (P<0.05). The pregnant women whose fetal heart rate was abnormal and the S/D≥3 had the highest fetal hypoxia incidence, followed by the pregnant women whose fetal heart rate was normal and the S/D≥3 and pregnant women whose fetal heart rate was abnormal and the S/D<3, sequentially. The fetal hypoxia incidence of pregnant women whose fetal heart rate was normal and the S/D<3 was the lowest. Conclusions Color Doppler ultrasound combined with fetal heart monitoring has important reference value in the diagnosis of fetal intrauterine hypoxia, and it is worthy of clinical applica
