经皮椎体后凸成形术球囊扩张体积对骨水泥注射剂量及邻椎再骨折的影响 被引量:8

摘要 目的探讨经皮椎体后凸成形术(PKP)球囊扩张体积对骨水泥注射剂量及邻椎再骨折的影响。方法回顾性分析2014年10月至2018年11月华北医疗集团峰峰总医院骨科收治的200例椎体压缩性骨折患者(均存在骨质疏松症)的病例资料,依据术后邻椎再骨折情况将患者分为两组:研究组100例,均发生邻椎再骨折;对照组100例,均未发生邻椎再骨折。观察两组患者的术后疼痛、球囊扩张体积、骨水泥注射剂量、X射线评分及运动功能情况。结果研究组患者球囊扩张体积为(3. 22±0. 30) ml,大于对照组的(2. 66±0. 51) ml,差异具有统计学意义(P <0. 05);研究组患者的骨水泥注射剂量为(3. 22±0. 54) ml,高于对照组的(2. 81±0. 25) ml,差异具有统计学意义(P <0. 05);两组患者的疼痛视觉模拟评分(VAS)(19. 27±6. 11 vs. 18. 24±4. 65分)、Oswestry功能障碍评分(3. 14±1. 27分vs. 2. 14±1. 59分)比较,差异无统计学意义(P> 0. 05);研究组患者的X射线评分高于对照组,差异具有统计学意义(P <0. 05)。结论经皮椎体后凸成形术中骨水泥注射过量会导致骨水泥渗漏,PKP球囊扩张体积越大,发生邻近椎体再骨折的概率越会增加,临床需要给予重视。 Objective To investigate the effect of percutaneous kyphoplasty(PKP)balloon dilation volume on bone cement injection dose and adjacent vertebral refracture.Methods Retrospective analysis of 200 cases of vertebral compression fractures(both osteoporosis)admitted to the Department of Orthopaedics,North China Medical Peak Peak Hospital from October 2014 to November 2018,based on postoperative adjacent vertebrae fractures The patients were divided into two groups:100 patients in the study group,with recurrent fractures of the adjacent vertebrae;100 patients in the control group,no adjacent fractures occurred.Postoperative pain,balloon dilatation volume,bone cement injection dose,X-ray score and motor function were observed in the two groups.Results The balloon dilation volume of the study group was(3.22±0.30)ml,which was greater than that of the control group(2.66±0.51)ml,and the difference was statistically significant(P<0.05).The bone cement injection dose of the study group was(3.22±0.54)ml,which was higher than the control group(2.81±0.25)ml,the difference was statistically significant(P<0.05).The pain visual analogue scale(VAS)of the two groups(19.27±6.11 piont vs.18.24±4.65 piont)and Oswestry function There was no significant difference in the score of the disorder(3.14±1.27 piont vs.2.14±1.59 piont)(P>0.05).The X-ray score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Bone cement injection dose can lead to bone cement leakage.The larger the PKP balloon dilation volume is,the higher the probability of adjacent vertebral body re-fracture will occur,which requires clinical attention.
