摘要
目的:探讨预开通腔内血管成形联合置管溶栓术治疗急性髂股静脉血栓形成临床疗效与安全性。方法:回顾性分析2015年11月—2017年12月于我院血管外科住院治疗的96例急性髂股静脉血栓形成患者的临床资料,根据导管溶栓前是否行小球囊预扩张分为观察组(42例)和对照组(54例),比较两组患者的血栓消除率以及围手术期症状性肺动脉栓塞、出血等并发症的发生情况。结果:全组患者技术成功率为100%,均顺利完成手术。观察组平均血栓清除率较对照组高、溶栓时间较对照组短(t_1=2.18,t_2=2.30,均P<0.01);两组间辅助腔内血管成形术率差异无统计学意义(χ~2=1.2,P>0.05)。两组术后均无症状性肺动脉栓塞病例发生。两组出血并发症发生率差异无统计学意义(χ~2=0.00,P>0.05)。结论:球囊预扩有助于缩短溶栓时间、提高血栓清除效果,且未增加症状性肺栓塞等严重并发症的发生率,在急性髂股静脉血栓形成的治疗中安全可靠。
Objective:To evaluate the clinical efficacy and safety of pre-open intravenous angioplasty combined with catheter thrombolysis in the treatment of acute iliofemoral vein thrombosis.Methods:The clinical data of 96 patients with iliofemoral vein thrombosis hospitalized in our hospital from November 2015 to December 2017 were retrospectively analyzed. According to whether balloon pre-dilation was performed before embolization catheter was placed,they were divided into observation group(42 cases) and control group(54 cases).Thrombosis elimination, symptomatic pulmonary embolism and bleeding complications were compared between the two groups.Results:The technical success rate of the whole group was 100%,and the operation was successfully completed.The average thrombus clearance rate and thrombolytic time in observation group were higher than those in control group(t1=2.18,t2=2.30,all P<0.01),but there was no significant difference in the rate of assisted intraluminal angioplasty between the two groups(χ2=1.2,P>0.05).There were no cases of asymptomatic pulmonary embolism after operation in both groups. There was no significant difference in the incidence of bleeding complications between the two groups(χ2=0.00,P>0.05).Conclusion:Pre-balloon dilation can shorten the time of thrombolysis and improve the effect of thrombolytic clearance without increasing the incidence of severe complications such as symptomatic pulmonary embolism.It is safe and reliable in the treatment of acute iliofemoral vein thrombosis.
