摘要
目的:探讨社区护理干预在老年高血压患者中的应用效果。方法:选取2016年8月-2017年10月上饶市信州区水南街街道的112例老年高血压患者作为研究对象,按照随机数字表法分为试验组与对照组,每组56例。对照组给予常规慢性疾病管理,试验组在对照组基础上给予社区护理干预。比较两组干预前后血压、生活习惯变化情况,同时比较两组患者对高血压知识的知晓率、遵医行为以及护理满意度。结果:干预后,试验组收缩压、舒张压均明显低于干预前与对照组,差异均有统计学意义(P<0.05);干预后,试验组吸烟控制率、饮酒控制率、食盐控制率以及体育锻炼率均明显高于干预前与对照组,差异均有统计学意义(P<0.05);试验组高血压知识总知晓率为92.86%(52/56),明显高于对照组的75.00%(42/56),差异有统计学意义(P<0.05);试验组总遵医率为96.43%(54/56),明显高于对照组的67.86%(38/56),差异有统计学意义(P<0.05);试验组护理总满意度为94.64%(53/56),明显高于对照组的78.57%(44/56),差异有统计学意义(P<0.05)。结论:对老年高血压患者实施社区护理干预,可明显改善患者血压水平与生活习惯,同时提高患者对高血压知识知晓率、遵医行为以及护理满意度,值得推广与应用。
Objective:To explore the effect of community nursing intervention in elderly hypertensive patients.Method:112 elderly hypertensive patients from Shuinan Street,Xinzhou District,Shangrao City,from August 2016 to October 2017 were selected as subjects,according to the random number table method,they were divided into experimental group and control group,56 cases in each group.The control group was given routine chronic disease management,the experimental group was given community care intervention on the basis of the control group.The changes of blood pressure and living habits before and after the intervention were compared between the two groups,meanwhile,the awareness rate of hypertension knowledge,compliance behavior and nursing satisfaction of the patients in the two groups were also compared.Result:After intervention,the systolic blood pressure and diastolic blood pressure in the experimental group were significantly lower than those before intervention and the control group,the differences were statistically significant(P<0.05).After the intervention,the ratio of smoking control rate,drinking control rate,salt control rate,and physical training rate in the experimental group were significantly higher than those before intervention and the control group,the differences were statistically significant(P<0.05).The total awareness rate of hypertension knowledge in the experimental group was 92.86%(52/56),which was significantly higher than 75.00%(42/56)in the control group,the difference was statistically significant(P<0.05);the total compliance rate of the experimental group was 96.43%(54/56),significantly higher than 67.86%(38/56)of the control group,the difference was statistically significant(P<0.05);the total nursing care satisfaction in the experimental group was 94.64%(53/56),significantly higher than 78.57%(44/56)of the control group,the difference was statistically significant(P<0.05).Conclusion:Community nursing intervention for elderly patients with hypertension can significantly improve patients’ bl
