摘要
目的比较2种腰椎关节手法对大鼠腰神经节损伤模型机械痛敏的影响,探讨不同力学形式手法的镇痛效果。方法72只雄性SD大鼠随机分为模型组、腰椎假手法组、腰椎松动手法组、腰椎调整手法组,每组各18只。其中模型组通过手术在特定椎节(L_(5-6))左侧建立L5背根神经节损伤模型;其余3个组则是在建模基础上,分别施行模拟L_(5-6)关节假手法、L_(5-6)关节松动手法、L_(5-6)关节调整手法。每组各取6只大鼠于术前1 d及术后第1、3、5、7、9、11、15、17、21天行50%机械性撤足痛阈值(PWT)的测定。结果假手法组50%PWT在术后第11、15、17天时高于模型组(P<0.05),21 d时更高(P<0.01)。调整手法组50%PWT在术后第7天时开始高于假手法组(P<0.05),第9、11、15、17、21天时更高(P<0.01)。松动手法组50%PWT在术后第1、3天时高于调整手法组(P<0.05),第5天时更高(P<0.01),但第7、9、11、15、17、21天时与调整手法组比较差异无统计学意义(P>0.05)。结论调整手法、松动手法均能产生比假手法更好的效果。其中松动手法作用柔和,手法刺激反应较小,在5 d内的效果优于调整手法;调整手法组作用剧烈,手法刺激反应较大,在7 d以后的效果与松动手法相近。
Objective To compare the effects of two methods of lumbar spinal manipulation of the lumbar facet joint on mechanical hyperalgesia in DRG in rats in order to explore the characteristics of different mechanical spinal manipulation methods in the treatment of acute neuropathic pain.Methods Seventy-two male SD rats were randomly divided into model group,sham manipulation group,mobilization group and thrusting manipulation group with 18 rats in each.All the rats received special surgery to establish a model and induce L5 nerve root inflammation.The model group was exposed to no interference during the observation.A model of acute left L5 nerve root inflammation in the specific L5-6 segment was established.The other three groups were treated with sham manipulation,mobilization,thrusting manipulation at the L5-6 joint on the basis of model establishment.Six rats were chosen from each to observe 50%mechanical paw withdrawal threshold(PWT)one day before modeling and 1,3,5,7,9,11,15,17,21 days after modeling.Results The 50%PWT in sham manipulation group was higher than that of model group 11,15 and 17 days after modeling(P<0.05),It was higher at 21 days(P<0.01).The 5%PWT in the manipulation group was higher than that in the sham manipulation group on the 7th day after operation(P<0.05),and higher on the 9,11,15,17,21 days(P<0.01).The 50%PWT of mobilization group was higher than that in thrusting manipulation group 1 and 3 days after modeling(P<0.05),especially 5 days after modeling(P<0.01),but there was no significant difference 7,9,11,15,17 and 21 days after modeling(P>0.05).Conclusion Thrusting manipulation and mobilization can both produce better effect than sham manipulation.The mobilization approach is less harmful than thrusting manipulation,with less intense response to stimulation and better effects within 5 days.Manipulation is a tough manual therapy,with bigger stimulus response and similar effects to mobilization within 7 days.
