Application of feedback system based on doctor-patient relationship in standardized residency training

摘要 目的观察以医患关系为基础的反馈体系,在住院医师规范化培训中的应用效果及价值。方法选取2018年1~9月广州中医药大学附属重庆北碚中医院90名住院医师为研究对象。采用数字表平均分配法,将上述所有住院医师平均归纳为观察组和对照组各45名。对照组为常规反馈体系,观察组为以医患关系为基础的反馈体系。使用配对t对检验比较2组医师综合评分的差异,使用χ~2检验比较2组医师医患纠纷发生率和培训效果的差异。结果观察组住院医师培训后总评分高于对照组[(95.05±1.28)分vs(74.68±0.32)分],差异具有统计学意义(t=13.571,P=0.000)。观察组住院医师培训医患纠纷发生率低于对照组(2.22%vs13.33%),差异具有统计学意义(χ~2=3.873,P=0.049)。观察组培训总有效率显著高于对照组(97.78%vs 82.22%),差异具有统计学意义(χ~2=6.049,P=0.014)。结论在住院医师规范化培训中应用以医患关系为基础的反馈体系,能够使住院医师加强对医患关系的重视,提高住院医师综合水平,减少医患纠纷发生,对提高临床住院医师规范化培训效果有重要作用。 Objective To observe the effect and value of feedback system based on doctor-patient relationship in standardized residency training. Methods Ninety residents of Chongqing Beibei Hospital of Traditional Chinese Medicine affiliated to Guangzhou University of Traditional Chinese Medicine were selected as subjects from January to September 2018. All the above residents were divided into observation group and control group, 45 in each group, by using the method of average distribution of digital tables. The control group was a routine feedback system, while the observation group was a feedback system based on doctor-patient relationship. The comprehensive score was tested by paired t-pair test. The incidence of doctor-patient disputes and the training effect were tested by χ^ 2. Results After training, the total score of the residents in the observation group was higher than that in the control group [(95.05±1.28) vs (74.68 ±0.32)], and the difference was statistically significant (t=13.571, P<0.001). The incidence of doctor-patient disputes in the observation group was lower than that in the control group (2.22% vs 13.33%), and the difference was statistically significant (χ^ 2 =3.873, P=0.049). The total effective rate of training in the observation group was significantly higher than that in the control group (97.78% vs 82.22%), and the difference was statistically significant (χ^ 2 =6.049, P=0.014). Conclusion The application of feedback system based on doctor-patient relationship in standardized residency training can make residents pay more attention to doctor-patient relationship, improve the comprehensive level of residents, reduce the incidence of doctor-patient disputes, and play an important role in improving the effect of standardized residency training.
