摘要
目的:探讨视频眼震电图在良性阵发性位置性眩晕(BPPV)患者诊断中的临床应用价值。方法:选择121例良性阵发性位置性眩晕患者,分别给予裸眼检查及视频眼震电图检查。观察比较不同类型的眼震检出率及临床疗效。结果:81例后半规管型患者中视频眼震电图检测出75例(92.59%),裸眼检测出63例(77.78%),两组比较差异有统计学意义(χ~2=0.116,P=0.017);25例前半规管型患者中视频眼震电图检测出23例(92.00%),裸眼检测出14例(56.00%),两组比较差异有统计学意义(χ~2=0.032,P=0.012);15例水平半规管型患者中视频眼震电图检测出14例(93.33%),裸眼检测出10例(66.67%),两组比较差异无统计学意义(χ~2=0.536,P=0.219)。经治疗,总有效率为119例(98.35%),其中后半规管型、前半规管型、水平半规管型临床有效率为98.77%、96.00%和100.00%,两两比较,差异无统计学意义(P>0.05)。结论:视频眼震电图可准确观察BPPV患者眼震发生情况,显著提高其眼震检出率。
Objective:This study aims to investigate the clinical value of video-electronystagmography(VEG)in benign paroxysmal positional vertigo(BPPV)patients.Methods:121 patients were examined by both naked eye examination and video-electronystagmography respectively.The different types of detection rate and clinical efficacy were observed and compared.Results:75 of 81 patients(92.59% )were detected in posterior semicircular canal type by VEG,63 of them(77.78% )by naked eye,with significant difference between the two groups(χ^2=0.116,P=0.017).23 of 25 patients(92.00% )were detected in anterior semicircular canal type by VEG,and 14 of them(56.00% )by naked eye,with significant difference between the two groups(χ^2=0.032,P=0.012).14 of 15 patients(93.33% )were detected in horizontal semicircular canal type by VEG,and 10 of them(66.67% )by naked eye,without significant difference between the two groups(χ^2=0.536,P=0.219).After treatment,the total effective rate was 119 cases(98.35% ).The clinical effective rates of posterior semicircular canal,anterior semicircular canal and horizontal semicircular canal were 98.77% ,96.00% and 100.00% respectively.Conclusion:VEG can accurately observe the occurrence of nystagmus in BPPV patients,significantly improve the detection rate,which is worth popularizing.
