摘要
目的:探讨心理护理联合运动疗法对妊娠期糖尿病(GDM)患者的效果及对心理状态和妊娠结局的影响。方法:将笔者所在医院2017年1月-2018年6月收治的58例GDM患者按照随机数字表法分为两组,对照组29例予以常规护理加饮食疗法,研究组29例予以心理护理联合运动疗法。比较两组患者临床疗效、血糖水平、心理状态及不良妊娠结局发生情况。结果:研究组患者临床总有效率(96.55%)显著高于对照组(68.97%),差异有统计学意义(P<0.05)。两组患者干预前空腹血糖、餐后2 h血糖水平、SDS、SAS评分比较,差异无统计学意义(P>0.05);干预后两组空腹血糖、餐后2 h血糖水平、SDS、SAS评分均较干预前显著降低,且研究组均低于对照组,差异均有统计学意义(P<0.05)。研究组不良妊娠结局发生率(6.90%)显著低于对照组(34.48%),差异有统计学意义(P<0.05)。结论:心理护理联合运动疗法在GDM患者中应用可有效控制患者血糖水平,改善患者心理状态,不良妊娠结局发生率较低。
Objective: To explore the effect of psychological nursing combined with exercise therapy on gestational diabetes mellitus(GDM) patients and its influence on psychological status and pregnancy outcome.Method:A total of 58 patients with GDM admitted to the hospital from January 2017 to June 2018 were divided into two groups according to random number method.29 patients in the control group were given routine nursing plus diet therapy,and 29 patients in the study group were given psychological nursing combined with exercise therapy.The clinical efficacy,blood glucose level,psychological status and adverse pregnancy outcomes were compared between the two groups.Result:The total clinical effective rate of the study group(96.55%) was significantly higher than that of the control group(68.97%),the difference was statistically significant(P<0.05).Fasting plasma glucose,2 h postprandial blood glucose level,SDS and SAS scores between the two groups were compared before intervention,the differences were not statistically significant(P>0.05).After intervention,fasting plasma glucose, 2 h postprandial blood glucose level,SDS and SAS scores of the two groups were significantly lower than those before intervention,and the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of adverse pregnancy outcomes in the study group(6.90%) was significantly lower than that in the control group(34.48%),the differences were statistically significant(P<0.05).Conclusion:Psychological nursing combined with exercise therapy in patients with GDM can effectively control the blood glucose level,improve the psychological status of patients,and the incidence of adverse pregnancy outcomes is low.
