摘要
国医大师周岱翰教授是我国著名中医肿瘤学专家,广州中医药大学首席教授,第三、四、六批全国老中医药学术经验继承指导老师。周老认为恶性肿瘤化疗导致的消化道不良反应总病机为胃气上逆致呕,湿浊下降而泄,牵连脾胃、大肠等脏腑。化疗过程中往往出现证候复杂化,脏腑气机紊乱,应该从脏腑功能角度把握其变化规律。以升降学说指导处方用药可以起到执简驭繁、提纲挈领的作用,治疗需注重"升降制衡""以和为用"。在升降学说指导下辨识化疗导致的消化道不良反应,体现了周岱翰教授以《黄帝内经》为源,《伤寒论》为宗的"天人合一"藏象整体观思维。通过追溯升降学说源流,总结周老对于恶性肿瘤化疗过程中脏腑升降规律的认识,依据"脾升胃降""肝升肺降""心肾相交""水火相济"等脏腑气机升降及药物升降浮沉特性,梳理周老临床实践中的常用药对半夏、黄连;旋复花、代赭石;春砂仁、白豆蔻;熟附子、生大黄等,并附典型医案一则。
Professor Zhou Daihan,the Chinese medicine master,is the eminent expert of oncology in TCM,the chief professor of Guangzhou University of TCM and the instructor of academic inheritance of national veteran experiences of TCM.Prof.Zhou believes the pathogenesis of digestive adverse reaction induced by chemotherapy in malignant tumor includes upward reversion of stomach qi,descending of damp,with spleen,stomach and large intestine involved.In chemotherapy,the symptoms are complex and zangfu qi activity is disorganized.Hence,the rule should be explored in view of zangfu functions.The ascending-descending theory guides the use of herbal formulas to achieve the harmonization in the body.On the base of the unity of heaven and human from Classic of Internal Medicine and Treaties on Febrile Disease,the holistic idea is used in the identification of the chemotherapy-induced digestive adverse reaction in guide by the ascending-descending theory.The origin of ascending-descending theory of zangfu organs were summarized,i.e.spleen ascending and stomach descending,liver ascending and lung descending,disharmony of heart and kidney or disharmony of water and fire,as well as the ascending,descending,floating and sinking features of herbal medications,the common herbs are pinellia ternate,coptis chinensis,inula flower,ruddle,fructus amomi,amomun kravanh,prepared radix aconite carmichaeli and raw rhubarb.The typical case was reported.
