纳米碳示踪技术联合动脉入路淋巴结分检用于直肠癌根治术中的效果观察

摘要 目的探讨纳米碳示踪技术联合动脉入路淋巴结分检用于直肠癌根治术的效果。方法回顾性分析2014年1月至2018年1月本院接受直肠癌根治术的58例患者临床资料,根据淋巴结分检方案将患者划分两组,其中观察组(n=33)采用纳米碳示踪技术联合动脉入路法分检淋巴结,对照组(n=25)采用传统方案分检淋巴结,比较两组淋巴结检出情况、阳性淋巴结检出情况及观察组中黑染与否的阳性淋巴结检出情况。结果观察组共检出淋巴结612枚(其中阳性56枚),其中166枚直径<5 mm(其中阳性38枚);对照组共检出淋巴结356枚(含阳性29枚),其中34枚直径<5 mm(其中阳性22枚)。观察组淋巴结检出数<12枚者少于对照组(P <0.05)。观察组淋巴结平均检出枚数、直径<5 mm淋巴结平均检出数、第一站及第二站淋巴结平均检出数多于对照组,差异均有统计学意义(均P <0.05),两组第三站淋巴结平均检出数比较,差异无统计学意义(P> 0.05)。观察组直径<5 mm阳性淋巴结平均检出数多于对照组,差异有统计学意义(P <0.05)。观察组黑染淋巴结阳性淋巴结检出率高于未被染色淋巴结,差异有统计学意义(P <0.05)。结论纳米碳示踪技术联合动脉入路淋巴结分检法用于直肠癌根治术,有助于提升淋巴结清扫及分检效果。 Objectives To investigate the value of carbon nanoparticles combined with arterial approach lymph node identification in radical resection of rectal cancer.Methods This was a retrospective analysis of clinical data of 58 patients who received radical resection of rectal cancer between January 2014 and January 2018.Patients were divided into treatment group and control group depending on the method of lymph node identification.Patient in the treatment group received carbon nanoparticles combined with arterial approach lymph node identification and those in the control group received conventional lymph node identification.Results of identification,pathological findings were compared between the two group.Rate of black stained positive lymph nodes was recorded in the treatment group.Results A total of 612 lymph nodes were identified in the treatment group,with 56 positive and 166 of diameter<5 mm(38 positive lymph node).A total of 356 lymph nodes were identified in the control group,with 29 positive and 34 of diameter<5 mm(22 positive lymph node).Number of patients with identified lymph node<12 was significantly lower in the treatment group than in control group(P<0.05).Average number of identified lymph nodes,number of identified lymph nodes of diameter<5 mm,and average number of identified lymph nodes at station 1 and 2 were all significantly larger in the treatment group than in the control group(P<0.05).Average number of identified lymph nodes at station 3 was comparable between the two group(P>0.05).Average number of identified positive lymph nodes of diameter<5 mm was significantly larger in the treatment group than in the control group(P<0.05).In the treatment group,rate of positivity was significantly higher in the black stained lymph nodes than in their counterpart(P<0.05).Conclusion Carbon nanoparticles combined with arterial approach lymph node identification could increase efficiency of lymph node dissection and identification in radical resection of rectal cancer.
