摘要
目的检测柯萨奇病毒抗体、自身抗体及心肌酶谱,并探讨其与扩张型心肌病的相关性。方法将宝鸡市中医医院2016年1月至2018年10月收治的89例扩张型心肌病患者作为观察组,另选择同期在我院体检的46例健康志愿者作为对照组。检测并比较两组受检者的柯萨奇病毒抗体、自身抗体表达[包括抗β1肾上腺素能受体自身抗体(ant-β1)、抗肌球蛋白重链自身抗体(ant-MHC)、抗毒蕈碱2受体自身抗体(ant-M2)、抗腺嘌呤核苷酸(ADP/ATP)转位酶(ANT)自身抗体(ant-ANT)]及心肌酶谱水平[包括肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、天门冬氨酸氨基转移酶(AST)和α-羟丁酸脱氢酶(α-HBDH)]。按NYHA心功能分级将观察组患者分为NYHAⅠ~Ⅱ级组(34例),NYHAⅢ~Ⅳ级组(55例),检测并比较两组患者的柯萨奇病毒抗体、抗心肌自身抗体表达以及心肌酶谱水平。结果观察组患者的Cox-IgM阳性率为34.83%,明显高于对照组的8.70%,其ant-β1、ant-MHC、ant-M2、antANT水平分别为(67.46±14.71)μg/L、(33.74±11.62)μg/L、(56.44±8.23)μg/L、(153.54±34.18)μg/L,均明显高于对照组的(48.78±10.03)μg/L、(21.58±9.51)μg/L、(30.11±6.21)μg/L、(67.59±19.43)μg/L,差异均有统计学意义(P<0.05);观察组患者的CK、CK-MB、AST和α-HBDH水平分别为(166.81±30.44) U/L、(26.74±4.18) U/L、(38.57±8.28) U/L、(175.36±41.28) U/L,均明显高于对照组的(103.46±23.15) U/L、(15.53±3.46) U/L、(22.69±7.44) U/L、(128.54±29.45) U/L,差异均有统计学意义(P<0.05);NYHAⅢ~Ⅳ级组患者的CoxB-IgM阳性率为43.64%,明显高于Ⅰ~Ⅱ级组的20.59%,其ant-β1,ant-MHC,ant-M2,ant-ANT水平分别为(70.48±17.15)μg/L、(36.58±14.19)μg/L、(61.25±9.68)μg/L、(176.54±40.29)μg/L,也均明显高于Ⅰ~Ⅱ级组的(62.55±13.03)μg/L、(30.66±10.53)μg/L、(52.18±7.69)μg/L、(143.58±26.43)μg/L,差异均有统计学意义(P<0.05);NYHAⅢ~Ⅳ级组的CK、CK-MB、AST和α-HBDH水平分别为(213.54±36.
Objective To detect the levels of coxsackievirus antibodies,autoantibodies,and myocardial zymogram, and to explore their association with dilated cardiomyopathy.Methods From January 2016 to October 2018,89 patients with dilated cardiomyopathy admitted to Traditional Chinese Medicine Hospital of Baoji City were selected as observation group,and 46 healthy volunteers who underwent physical examination in our hospital during the same period were selected as control group.The expression of coxsackievirus antibodies,autoantibodies and myocardial zymogram were compared and analyzed between observation group and control group.Autoantibodies included anti-β1-adrenergic receptor autoantibodies (ant-β1),anti-myosin heavy chain (ant-MHC) autoantibodies,anti-muscarinic M2 receptor autoantibodies (ant-M2),anti-adenine nucleotide (ADP/ATP) translocase (ANT) autoantibodies (ant-ANT).Myocardial zymograms included creatine kinase (CK),creatine kinase-MB (CK-MB),aspartate aminotransferase (AST),and alpha- hydroxybutyrate dehydrogenase (α-HBDH).The patients in the observation group were divided into NYHA Ⅰ~Ⅱ group (34 cases) and NYHA Ⅲ~Ⅳ group (55 cases) according to NYHA cardiac function classification.The expression of coxsackievirus antibodies,autoantibodies and myocardial zymogram were compared and analyzed between NYHA Ⅰ~Ⅱ group and NYHA Ⅲ~Ⅳ group.Results In the observation group,the positive rate of Cox-IgM was 34.83%, which was significantly higher than 8.70% in the control group;the levels of ant-β1,ant-MHC,ant-M2,and ant-ANT were (67.46 ± 14.71)μ g/L,(33.74 ± 11.62)μ g/L,(56.44 ± 8.23)μ g/L,(153.54 ± 34.18)μ g/L,significantly higher than (48.78±10.03)μg/L,(21.58±9.51)μg/L,(30.11±6.21)μg/L,(67.59±19.43)μg/L in the control group (P<0.05).The levels of CK,CK-MB,AST,alpha-HBDH were (166.81±30.44) U/L,(26.74±4.18) U/L,(38.57±8.28) U/L,(175.36±41.28) U/L in the observation group,significantly higher than (103.46±23.15) U/L,(15.53±3.46) U/L,(22.69±7.44) U/L,(128.54± 29.45) U/L in the co
