咪达唑仑联合芬太尼辅助机械通气治疗重症肺炎患儿的镇痛效果 被引量:2

Analgesic Effect of Midazolam Combined with Fentanyl Assisted Mechanical Ventilation in Children with Severe Pneumonia

摘要 目的探讨咪达唑仑联合芬太尼辅助治疗机械通气重症肺炎患儿的效果。方法选择我院2016年7月~2018年7月60例行机械通气治疗的重症肺炎患儿,随机分为观察组与对照组,各30例。对照组单独应用咪达唑仑镇静镇痛,观察组在此基础上联合芬太尼镇静镇痛,比较两组患儿的机械通气时间、住院时间、用药后6、12、24 h的心率、血压、Ramsay评分及FLACC评分。结果观察组机械通气时间及住院时间短于对照组,差异有统计学意义(P<0.05);用药后6、12、24 h观察组患者心率、血压均低于对照组,差异有统计学意义(P<0.05);用药后6、12、24 h观察组Ramsay评分高于对照组, FLACC评分低于对照组,差异均有统计学意义(P<0.05)。结论临床上采取咪达唑仑联合芬太尼的,镇静镇痛效果均能明显提升,可稳定血流动力学,缩短患者苏醒时间和住院时间。 Objective To investigate the effect of midazolam combined with fentanyl on mechanical ventilation in children with severe pneumonia. Methods 60 children with severe pneumonia who underwent mechanical ventilation from July 2016 to July 2018 were randomly divided into observation group and control group, 30 cases each. In the control group, midazolam was used for sedation and analgesia. The observation group was combined with fentanyl for sedation and analgesia. The mechanical ventilation time, hospitalization time, heart rate at 6, 12, and 24 h after treatment were compared between the two groups. Blood pressure, Ramsay score and FLACC score. Results The mechanical ventilation time and hospitalization time in the observation group were shorter than those in the control group,the difference was statistically significant (P<0.05). The heart rate and blood pressure of the observation group were lower than those of the control group at 6, 12, and 24 h after treatment,the difference was statistically significant.(P<0.05);the Ramsay score of the observation group was higher than that of the control group at 6, 12, and 24 h after treatment, and the FLACC score was lower than that of the control group, the difference was statistically significant (P<0.05).Conclusion When midazolam combined with fentanyl is used, the sedative and analgesic effects can be significantly improved, which can stabilize hemodynamics and shorten the recovery time and hospitalization time.
