摘要
目的探讨基层社区利用慢病APP平台进行慢病健康管理的内容、步骤和规范,评估慢病APP健康管理后对改善患者血压的效果。方法 2018年1～6月,选取清远市清新区太和镇及太平镇322例高血压病患者为研究对象,将其随机分为对照组(传统高血压管理模式组)和观察组(采用标准化的慢病APP管理模式组)每组各166例;应用慢病APP管理模式将两组患者前后的患者性别、年龄、文化程度、BMI值、相关知识知晓率、生活方式、服药治疗情况、危险级别及管理级别进行比较。结果观察组的文化程度、BMI值、相关知识知晓率、生活方式、服药治疗情况及管理级别水平均高于对照组,差异均有统计学意义(P<0.01);影响慢病APP管理模式效果的因素有年龄、文化程度、体重指数、相关知识知晓率、生活方式、服药治疗情况及管理级别。结论采用标准化的慢病APP管理模式在高血压管理中的血压控制效果明显,为基层社区采用多元化管理高血压患者提供借鉴作用。
Objective To investigate the contents, procedures and standards in management of chronic diseases on community basis using chronic disease management APP, and assess the effects on the patients with hypertension following such health management. Methods 322 hypertension patients were recruited from Thaihe and Taiping towns, Qingxin district of Qingyuan city between January and June of 2018, and equally randomized into control group (traditional hypertension management mode) and observational group (standardized chronic disease APP management mode). Then the two groups were compared based on the chronic disease APP management mode, regarding the patient gender, ages and educational background as well as body mass index(BMI), relevant knowledge and awareness on hypertension, changes of life style, medication compliance, risk level and management level before and after management. Results The educational background, BMI, rate of knowledge and awareness on hypertension, changes of life style, medication compliance and management level were higher in the observational group than in the control group, with significant difference( P <0.01). Factors affecting the effect of APP management mode for chronic diseases included the age, educational background, BMI, degree of knowledge and awareness on hypertension, changes of life style, medication compliance and management level. Conclusion Standardized chronic disease APP management mode can be effective in control of hypertension on community basis by diversified management of hypertension patients.
