摘要
目的分析酒精性肝硬化患者的糖脂代谢与肝功能Child-Pugh分级的关系。方法 49例酒精性肝硬化患者作为试验组,根据肝功能Child-Pugh分级将其分为A级组、B级组和C级组,另选25例无肝脏病史、无糖脂代谢异常的健康男性作为对照组,对比分析各组间糖脂代谢指标的差异。结果试验组空腹血糖、餐后2h血糖、餐后2h胰岛素、胰岛素释放指数、糖化血红蛋白显著高于对照组,胰岛素敏感指数显著低于对照组(P均<0.05),A组、B组和C组间空腹血糖、空腹胰岛素、餐后2h血糖、餐后2h胰岛素、胰岛素释放指数、胰岛素敏感指数和糖化血红蛋白组间存在差异(P均<0.05)。试验组低密度脂蛋白胆固醇较对照组低(P<0.05),A组、B组和C组间总胆固醇和低密度脂蛋白胆固醇存在差异(P均<0.05)。结论酒精性肝硬化患者存在糖脂代谢异常,随着肝功能Child-Pugh分级增高而更加突出。
Objective This paper intends to analyze the relationship between glucose and lipid metabolism and Child-Pugh degree in patients with alcoholic cirrhosis. Methods 49 patients with alcoholic cirrhosis were assigned to group A, group B, and group C according to the Child-Pugh degree, and 25 healthy men with no history of liver disease and no abnormalities in glycolipid metabolism were used as a control group. Statistic analysis was performed in each group and between groups. Results Patients with alcoholic cirrhosis had a higher level of fasting plasma glucose, 2-hour postprandial plasma glucose, 2-hour postprandial insulin, insulin release index, glycosylated hemoglobin Alc than the people in the control group, but had a lower level of insulin sensitivity index (all P <0.05). There were differences in fasting plasma glucose, fasting insulin, 2-hour postprandial plasma glucose glucose, 2-hour postprandial insulin, insulin release index, insulin sensitivity index, and glycosylated hemoglobin Alc among the groups (all P <0.05). Alcoholic cirrhosis patients had a lower level of low density lipoprotein cholesterol ( P <0.05). There were differences in total cholesterol and low density lipoprotein cholesterol among the groups (all P <0.05). Conclusions Patients with alcoholic cirrhosis had abnormal glucose and lipid metabolism. With the increase of Child-Pugh degree, metabolic disorders of glucose and lipid worsen.
