摘要
目的对比分析腹腔镜与显微镜下精索内静脉高位结扎术治疗精索静脉曲张的临床效果。方法研究时间段为2017年1月至2018年3月,以随机数字表法于期间抽取本院收治的精索静脉曲张患者94例为对象,再将其以单盲法分为两组:对照组患者接受腹腔镜下精索内静脉高位结扎术,观察组患者接受显微镜下精索内静脉高位结扎术。对比两组治疗前后精子质量(精子密度/SC、精子活力/SM、精子畸形率/SDR)、体内性激素水平(黄体生成素/LH、促卵泡激素/FSH、睾酮/T、游离睾酮/FT),采用国际勃起功能指数(IIEF-5)评价勃起功能变化情况,视觉模拟评分(VAS)评价阴囊疼痛情况。结果①治疗后观察组患者SC、SM、SDR数值比对照组优(P<0.05);②治疗后两组患者体内LH、FSH、T、FT等性激素水平数值与治疗前比较差异具有统计学意义;③治疗后两组IIEF-5积分与治疗前比较差异具有统计学意义,但对照组患者阴囊VAS评分比观察组患者低,差异具有统计学意义(P<0.05)。结论精索静脉曲张患者接受显微镜下精索内静脉高位结扎术治疗的效果更佳。
Objective To compare and analyze the clinical effect of high ligation of internal spermatic vein under laparoscope and microscope in the treatment of varicocele. Methods From January 2017 to March 2018, 94 patients with varicocele were selected from our hospital by random digital table. The patients in the control group received laparoscopic high ligation of the internal spermatic vein, and the patients in the observation group received the high ligation of the spermatic vein under microscope. Compared two groups of sperm quality before and after the treatment (sperm density, sperm vitality/SM/SC/SDR, sperm deformity rate) and sex hormone levels in the body (luteinizing hormone/LH/FSH, follicle stimulating hormone, testosterone, free testosterone/T/FT), USES the international index of erectile function (IIEF-5) evaluation of erectile function and visual analogue scale (VAS) evaluation of scrotal pain. Results The value of SC, SM, SDR in the observation group was better than that in the control group after treatment (P<0.05). There were significant differences in the levels of LH, FSH, T, FT and other sex hormones between the two groups after treatment and before treatment. There was a significant difference in IIEF-5 scores between the two groups after treatment and before treatment, but in the control group, there was a significant difference between the two groups. The VAS score of scrotum was significantly lower than that of observation group (P<0.05). Conclusion High ligation of internal spermatic vein under microscope is more effective in patients with varicocele.
