骨瓣开颅治疗矢状窦区颅骨凹陷性骨折12例

摘要 目的:总结矢状窦区凹陷性骨折骨瓣开颅12例的手术治疗经验。方法:选取矢状窦区凹陷性骨折的患者12例,均采取骨瓣开颅进行治疗,完全游离出带有骨折碎片的骨瓣,然后进行修复,同时进行清创,摘除颅脑内的骨折碎片,清除颅内的血肿,修好破裂的硬脑膜和矢状窦,把整复好的骨瓣进行复位固定。结果:所有患者术后头颅三维CT复查结果均显示凹陷性骨折基本获得解剖上的复位,未出现术后伤口感染,预后良好。结论:骨瓣开颅是治疗矢状窦区颅骨凹陷性骨折的有效方法。 Objective: To summarize our experience with bone flap craniotomy in 12 cases of depressed skull fracture in the superior sagittal sinus area. Methods: For this study, we recruited 12 patients who were diagnosed with depressed skull fractures in the superior sagittal sinus area. All patients were treated with bone flap craniotomy. During the treatment process, the bone flap with fracture fragments was completely dissociated from the brain and repaired;meanwhile, debridement was performed to remove fracture fragments in the brain and eliminate intracranial hematoma. The cracked endocranium and superior sagittal sinus were repaired, and the repaired bone flap was restored and fixed. Results: Cranial three-dimensional CT scans for all patients after operation showed that depressed fractures were anatomically restored. Post-operation wound infection did not occur, and prognosis was excellent. Conclusion: Bone flap craniotomy is an effective treatment for depressed skull fractures in the superior sagittal sinus area.
