硫酸镁对妊娠期高血压患者血流动力学、产后出血和新生儿Apgar评分的影响 被引量:7

Effects of Magnesium Sulfate on Hemodynamics,Postpartum Hemorrhage and Neonatal Apgar Score in Patients with Pregnancy Induced Hypertension

摘要 目的探讨硫酸镁对妊娠期高血压疾病患者血流动力学、产后出血和新生儿Apgar评分的影响。方法选取2016年1月至2018年7月期间广州市中西医结合医院产科收治的106例妊娠期高血压疾病患者为研究对象,分为对照组和观察组,各组53例,对照组患者给予常规降压及支持治疗,观察组在对照组的基础上加用硫酸镁治疗,对比分析两组患者血流动力学指标、产后出血情况和新生儿Apgar评分。结果给予硫酸镁治疗后,观察组患者血流动力学指标如血液黏度、红细胞压积、RI以及S/D较对照组患者得到明显改善(P<0. 05);观察组产后出血发生率3. 77%,产后24 h阴道总出血量(85. 42±20. 61) m L,低于对照组(P<0. 05);观察组新生儿娩出1. 0 min时及5. 0 min时Apgar评分分别为(7. 05±1. 22)分和(9. 17±0. 27)分,显著高于对照组(P<0. 05)。结论采用硫酸镁治疗妊娠期高血压疾病可改善血流动力学和母婴结局。 Objective To investigate the effect of magnesium sulfate on hemodynamics,postpartum hemorrhage and neonatal Apgar score in patients with pregnancy induced hypertension.Methods 106 cases of pregnancy-induced hypertension(PIH)admitted to Obstetrics Department of Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2016 to July 2018 were selected as the subjects.The subjects were divided into the control group and the observation group,with 53 cases in each group.Patients in the control group received conventional antihypertensive therapy and supportive treatment,while those in the observation group were additionally treated with Magnesium Sulfate on the basis of the control group.Hemodynamic parameters,postpartum hemorrhage and neonatal Apgar score were compared between the two groups.Results After treatment with magnesium sulfate,the hemodynamic indexes such as blood viscosity,hematocrit,RI and S/D in the observation group were significantly improved compared with those in the control group(P<0.05).The incidence of postpartum hemorrhage in observation group was 3.77%.The total amount of vaginal hemorrhage in 24 hours after delivery was(85.42±20.61)mL,which was lower than that in control group(P<0.05).The Apgar scores at 1.0 min and 5.0 min of delivery in the observation group were(7.05±1.22)and(9.17±0.27)respectively,which were significantly higher than those in the control group(P<0.05).Conclusions Magnesium sulfate can improve hemodynamics and maternal-infant outcomes in the treatment of pregnancy-induced hypertension.
