单个心动周期冠状动脉CT血管成像对左心室心肌密度的测量研究 被引量:2

摘要 目的:研究正常人左心室心肌密度在收缩末期(end systolic, ES)与舒张末期(end diastolic, ED)心内膜下层和心外膜下层心肌密度(CT值)分布的差异。方法 :选择冠状动脉CT血管成像(CT angiography, CTA)检查中,单个心动周期内进行多期相扫描(5%~95%)的冠状动脉正常的患者共18例,应用CardIQ Xpress软件自动找出最佳ES与ED时相的图像,在四腔心层面,二尖瓣与心尖连线的垂直位置重建标准左心室短轴位图像,层厚4 mm,根据美国心脏学会(American Heart Association, AHA)心脏短轴面16节段模型,分别测量心内膜下层和心外膜下层心肌密度的分布。用配对t检验和独立样本t检验比较其差异。结果:同一节段的ES或ED的心内膜下层与心外膜下层CT值比较差异有统计学意义(P<0.05)。ES与ED心内膜下层16节段的CT值比较差异无统计学意义(P>0.05),但ES与ED心肌外膜下层的16节段心肌CT值比较差异有统计学意义(P<0.05)。结论:采用单个心动周期测量心内膜下层和心外膜下层心肌的密度是可行的,所研究的方法和结果可用于指导对心肌活性评估时心肌密度测量的研究。 Objective : To study the attenuation differences of myocardial density(CT value) in left ventricular myocardium of the subepicardium and the subendocardium at the end systolic(ES) and end diastolic(ED) in normal patients. Methods : Eighteen patients with normal coronary artery had multiphase phase scanning(5%-95%) in single cardiac cycle before and after contrastenhancement in coronary CT angiography,were enrolled for this study. To detect the best ES and ED images automatically by CardIQ Xpress software. To reconstruct the standard left ventricular short axis images in the vertical position of mitral and apical connections at the four chambers level,with a thickness of 4 mm. According to the American heart association 16-segment model of myocardial segments,the density of the subendocardial and the subepicardial myocardium were measured,respectively. A paired Student t test and Independent sample t test were used for the comparison of CT values indifferent segment of myocardium. Results : To compare CT values between the same segment of subendocardial and subepicardial myocardium in the same ES or ED respectively(P<0.05). To compare CT values between ES and ED in the subendocardial or subepicardial myocardium respectively,there was no statistical significance in the study of the 16 segments of the subendocardial myocardium(P>0.05). But the subepicardial myocardium was statistically significant(P<0.05). Conclusions : It is feasible to use ED and ES for measuring the density of subendocardial and subepicardial myocardium by single cardiac cycle scan. The methods and results of this study can be used to guide myocardial density measurement while evaluating myocardial viability.
