多b值弥散加权成像术前预测垂体瘤质地的研究 被引量:2

摘要 目的探讨多b值磁共振弥散加权成像(diffusion weighted imaging,DWI)及表观扩散系数(apparent diffusion coefficient,ADC)术前预测垂体腺瘤质地的应用价值。方法选取57例术后病理证实为垂体腺瘤的患者纳入研究。所有患者均在术前进行3. 0 T MRI多b值DWI扫描(b=0,b=1 000 s/mm^2,b=2 000 s/mm^2),以及内分泌激素检查。选取两种b值(b=1 000、2 000 s/mm^2)重建ADC图,并获取肿瘤实质区域的ADC值。根据术中肿瘤质地将患者分为质软组及质韧组;比较两种b值下,质软组与质韧组ADC均值的差异性;并分析肿瘤质地与患者性别、年龄、激素分型及肿瘤胶原含量的相关性。结果 b1000中,质软组与质韧组ADC均值的差异无统计学意义(P> 0. 05); b2000中,两组间ADC均值的差异有统计学意义(P <0. 05)。在b1000和b2000中,肿瘤质地与患者年龄、性别及激素分型均无明显相关性;而质软组的肿瘤胶原含量均明显低于质韧组(均P <0. 05)。结论垂体腺瘤的质地与ADC值及肿瘤胶原含量有密切关系,与患者的性别、年龄及激素分型无关。DWI和ADC可在术前预测肿瘤质地,为手术入路的选择提供影像学依据。 Objective To explore the application value of Multiple b-Value DWI and ADC in predicting the consistency of pituitary adenomas before operation.Methods 57 patients with pituitary adenoma confirmed pathologically were included in the research.All patients underwent examination,which included 3.0 T MRI multiple b-value ( b =0,1 000,2 000 s/mm 2) DWI scan and endocrine hormone.Volumes of interest (VOIs) were drawn in the pituitary adenomas on ADC maps generated with two b-values ( b =1 000,2 000 s/mm 2).Consistency of pituitary adenomas was evaluated at surgery and classified as soft group or hard group.The relationship between the consistency of tumor with mean ADC values,patients' age,gender,hormone classification and collagen contents in tumors were analyzed under two b values.Results There was no statistically significant difference in b1000( P <0.05) but lots of significant difference in b2000 ( P >0.05) between the tumor consistency and mean ADC values.No other obvious correlations were found between tumor consistency with the patients' age,gender or hormone classification in neither b1000 nor b2000, but tumor collagen content of soft group were significantly lower than the hard group (both P <0.05).Conclusions Tumor consistency of pituitary adenomas has close relationship with mean ADC values and collagen contents in tumors but has no relationship with the patient's gender,age or hormone classification.DW MR images with ADC values can be used to predict the consistency of Pituitary adenomas,which provide important imaging basis for the choice of surgical approach.
