摘要
目的分析江西地区细胞色素P4502C19(CYP2C19)基因型的分布情况。方法采用等位基因特异PCR结合荧光探针技术,对3 408例患者进行CYP2C19~*1、~*2、~*3基因检测。结果 CYP2C19~*1、~*2、~*3的基因频率分别为63.3%、32.0%、4.7%;而CYP2C19~*1/~*1、CYP2C19~*1/~*2、CYP2C19~*1/~*3、CYP2C19~*2/~*2、CYP2C19~*2/~*3、CYP2C19~*3/~*3的频率分布分别为40.1%、40.5%、5.9%、10.3%、3.0%、0.3%;其快代谢型、中代谢型、慢代谢型的频率分别为40.1%、46.4%、13.5%;不同性别之间CYP2C19基因型及代谢型差异均无统计学意义(P>0.05),各年龄段各代谢型差异也无统计学意义(P>0.05)。结论江西地区心脑血管患者CYP2C19基因型主要以CYP2C19~*1/~*2为主,代谢表型以中代谢为主,可以此评估其氯吡格雷抵抗风险,从而精准地为患者制定个体化医疗方案,更大程度地降低不良心脑血管事件的发生。
Objective To analyze the genotype distribution of cytochrome P4502 C19(CYP2 C19)in Jiangxi province.Methods By alleles specific PCR combined with fluorescence probe technique,A total of 3 408 patients were detected CYP2 C19*1,*2 and*3 allele.Results The allele frequencies of CYP2 C19*1,*2 and*3 were 63.3%,32.0%and 4.7%respectively;and the distribution of CYP2 C19*1/*1,CYP2 C19*1/*2,CYP2 C19*1/*3,CYP2 C19*2/*2,CYP2 C19*2/*3,CYP2 C19*3/*3 were 40.1%,40.5%,5.9%,10.3%,3.0%,0.3%respectively;and the distribution of extensive metabolilism.intermediate metabolilism and poor metabolilism were 40.1%.46.4%,13.5%;The CYP2 C19 genotypes and metabolilism had no statistical difference in different genders(P>0.05);and also had no statistical difference in different age groups(P>0.05).Conclusion The major genotype of patients with cardiovascular and cerebrovascular diseases is CYP2 C19*1/*2 and the major metabotropic is intermediate metabolilism in Jiangxi province,then we can evaluate the clopidogrel resistance risk and formulate the personalized therapeutic scheme accurately,it will greatly reduce the probability of adverse medical events.
