摘要
目的探讨我国天津市某社区卫生服务中心建卡孕妇妊娠糖尿病发病的相关危险因素。方法回顾性分析2015年1月1日至2018年6月25日天津市某社区卫生服务中心建卡孕妇信息,初步分析妊娠糖尿病患病率及患病危险因素。结果天津市某社区卫生服务中心建卡孕妇妊娠糖尿病的患病率为23%(414/1798,95%CI:21%~25%);妊娠糖尿病发生相关因素中,年龄、孕早期空腹血糖(fasting plasma glucose,FPG)升高、孕早期体质指数(body mass index,BMI)较高均为妊娠糖尿病的危险因素(OR_(年龄)=1.04,95%CI:1.01~1.08;OR_(FPG)=2.82,95%CI:2.13~3.73;OR_(超重)=1.91,95%CI:1.44~2.52;OR_(肥胖)=2.46,95%CI:1.70~3.56)。妊娠过程中,妊娠糖尿病孕妇的收缩压(systolic blood pressure,SBP)和BMI高于非妊娠糖尿病孕妇(F_(SBP)=49.29,P<0.01;F_(BMI)=41.69,P<0.01);妊娠糖尿病孕妇的BMI较高趋势持续至分娩结束(t_(BMI)=5.45,P <0.01),SBP较高趋势持续至孕28周,两组SBP于产后42d接近(t_(SBP)=0.02,P=0.98)。结论天津市某社区卫生服务中心的建卡孕妇妊娠糖尿病呈高水平流行;妊娠糖尿病发病危险因素有高龄、孕早期FPG升高、孕早期超重/肥胖。
Objective To explore the risk factors of gestational diabetes mellitus(GDM)in pregnant women in a community health center in Tianjin.Method Retrospective analysis of pregnant women’s information on a community health center in Tianjin from January 1,2015 to June 25,2018,and preliminary analysis of the prevalence of GDM and risk factors.Result The prevalence of GDM in pregnant women in a community health center in Tianjin was 23%(414/1798,95%CI:21%~25%).Among the factors related to GSM,high age,early pregnancy fasting plasma glucose(FPG),and early body mass index(BMI)were risk factors for GDM(ORage=1.04,95%CI:1.01~1.08;ORFPG=2.82,95%CI:2.13~3.73;ORoverweight=1.91,95%CI:1.44~2.52;ORobesity=2.46,95%CI:1.70~3.56).During pregnancy,systolic blood pressure(SBP)and BMI were higher in pregnant women with GDM than that in non-GDM(FSBP=49.29,P<0.01;FBMI=41.69,P<0.01).The higher BMI trend in pregnant women with GDM lasts until the end of childbirth(tBMI=5.45,P<0.01).The higher trend of SBP in pregnant women with GDM persisted until 28 weeks of gestation,and the SBP of the two groups was close at 42 days postpartum(tSBP=0.02,P=0.98).Conclusion GDM in pregnant women in a community health center in Tianjin is highly prevalent.The risk factors for GDM include advanced age,fasting fasting blood glucose in early pregnancy,and overweight and obesity in early pregnancy.
