摘要
目的探讨超声引导下不同腰丛定位法在老年髋关节术中应用效果。方法选择43例髋关节术老年患者,根据入院时间段不同分为观察组与对照组。对照组采用超声引导下纵截面平面外法定位,施行腰丛神经阻滞,观察组采用超声引导下三叶草法定位,施行腰丛神经阻滞,对比两组的运动、感觉神经阻滞起效时间和维持时间,术后不同时间点疼痛VAS评分及术后不良反应。结果观察组的运动、感觉神经阻滞起效时间均明显短于对照组,维持时间均明显长于对照组(P <0.05);两组术后不同时间点VAS评分及不良反应无统计学差异(P> 0.05)。结论三叶草法与纵截面平面外法两种超声引导定位法下施行腰丛神经阻滞置换老年髋关节,术后镇痛效果、安全性相当,但三叶草法的阻滞麻醉效果优于纵截面平面外法。
Objective To explore the effect of ultrasound-guided lumbar plexus localization in the hip joint surgery of senile patients.Methods A total of 43 senile patients with hip joint surgery were divided into an observation group and a control group according to the time of admission.The control group received ultrasound-guided out-of-plane localization and lumbar plexus nerve block.The observation group received ultrasound-guided clover localization and lumbar plexus nerve block.The onset and maintenance time of motor and sensory nerve block were compared between the two groups.The VAS score of pain and adverse reactions at different time points after operation were compared.Results The onset time of motor and sensory nerve block in the observation group was significantly shorter than that in the control group,and the maintenance time was significantly longer than that in the control group(P<0.05);there was no significant difference in VAS scores and adverse reactions between the two groups at different time points after operation(P>0.05).Conclusion Lumbar plexus nerve block for hip joint replacement in senile patients under two ultrasound-guided localization methods,i.e.,clover method and longitudinal section out-of-plane method,has the same analgesic effect and safety,but the blocking anesthesia effect of clover method is better than that of longitudinal section out-of-plane method.
