摘要
目的:比较机器人辅助腹腔镜肾部分切除术与传统术式的手术治疗及康复情况。方法:回顾性分析南京大学医院附属鼓楼医院2015年1月至2017年7月行肾部分切除术患者的临床资料,方便选取行机器人辅助腹腔镜肾部分切除术的患者50例为观察组,行传统术式的患者50例为对照组。比较两组患者的手术治疗情况及术后康复效果。结果:观察组在手术时间、术中出血量、术后镇痛药物使用量、术后首次下床时间、肛门排气时间、拔管时间、术后住院时间方面优于对照组,但在手术费用上高于对照组,差异有统计学意义(P<0.05);在术中输血率、术后严重并发症的发生率方面两组差异无统计学意义(P>0.05)。结论:机器人辅助腹腔镜肾部分切除术具有创伤小、出血少、操作精准、恢复快等优点,缩短了住院时间,降低了并发症的发生率,利于患者康复,值得临床大力推广。
Objective:To investigate and analyze the clinical effect of robot-assisted laparoscopic partial nephrectomy and laparoscopic partial nephrectomy.Methods:The data of 100 patients with renal tumor who underwent partial nephrectomy from January 2015 to July 2017 in Nanjing Drum Tower Hospital Affiliated to Nanjing University Medical School were collect and analyzed.50 patients underwent robot-assisted laparoscopic partial nephrectomy(observation group)while the other 50 patients underwent laparoscopic partial nephrectomy(control group).The surgical treatment of the patients were compared between the two groups and postoperative rehabilitation effect.Results:The operation time,intraoperative blood loss,postoperative analgesic use,postoperative first ambulation time,anal exhaust time,extubation time of drainage tube and indwelling catheter and postoperative hospitalization time of the observation group were better than the control group,but the cost of operation is higher than that of control group,the differences were statistically significant(P<0.05),there was no significant difference between the robotic group and the laparoscopic group in the rate of blood transfusion and the incidence of postoperative severe complications(P>0.05).Conclusion:Robot-assisted laparoscopic partial nephrectomy has the advantagesof less trauma,less bleeding,accurate operation,rapid recovery,shorten the length of hospital stay,reduce the incidence of complications,and promote the rehabilitation of patients,and deserve clinical promotion.
