摘要
目的探究低磁场共振成像技术(MRI)在乳腺癌早期诊断及分期中的作用。方法选取2017年4月至2018年5月湖南市中心医院收治的100例乳腺癌患者,采用低MRI技术检查,记录检查结果,然后和病理学诊断结果进行对比,评判MRI技术在乳腺癌早期诊断和分期判断中的准确性。结果①100例研究对象经组织病理学检查共检出116个病灶,其中恶性病灶67个,占57.76%,良性病灶49个,占42.24%。MRI技术共检出恶性病灶67个,符合率为100%,但是有2例脂肪瘤未经MRI检出;②MRI技术对直径在3.50cm以上的肿块体积检查误差较小,但是对于直径<3.50cm的肿块,MRI技术测得的肿块直径(r)与实际肿块的直径(R)相对误差要>15.00%;③共64例毛齿特征的乳腺肿块,MRI检出58例(90.63%),有6例边缘毛齿特征的乳腺肿块没有被检出,MRI技术对原位癌(TIS)的识别率较低,15例共检出9例,但是对于T2、T3、T4分期的乳腺癌检出率较高,为100%。T1期有4例未被MRI检出。结论低MRI技术对于体积较大的恶性乳腺肿块检出具有非常高的准确率,但是对于体积较小、原位癌的检出能力有限,在乳腺癌的早期筛查过程中,应充分结合病理学检查结果进行诊断。
Objective To investigate the role of low magnetic resonance imaging (MRI) in the early diagnosis and staging of breast cancer.Methods A total of 100 patients with breast cancer were enrolled,and the results of low MRI were recorded for comparison with pathological findings to evaluate the accuracy of MRI in the early diagnosis and staging of breast cancer.Results A total of 116 lesions were detected by histopathological examination in 100 subjects,including 67 malignant lesions,accounting for 57.76%,and 49 benign lesions,accounting for 42.24%.A total of 67 malignant lesions were detected by MRI technique,and the coincidence rate was 100%.But 2 cases of lipoma were not detected by MRI.MRI technique had a small error in measuring the volume of tumors with a diameter of more than 3.50cm,but for a mass of <3.50cm in diameter,the relative error of the diameter (r) of the mass measured by MRI technique and the diameter (R) of the actual mass was greater than 15.00%.A total of 64 breast lesions with dentate features were detected by MRI in 58 cases (90.63%).Six cases with marginal dentate features were not detected.MRI technique had a lower recognition rate for carcinoma in situ.A total of 9 cases of 15 were detected,but the detection rate of breast cancer in T2,T3,and T4 stages was higher (100%).Four patients in T1 stage were not detected by MRI.Conclusion Low MRI technique has a very high accuracy for the detection of large malignant breast masses.However,for small tumors and carcinoma in situ,diagnosis should be given by fully combining pathological findings in the early screening of breast cancer.
