摘要
目的通过回顾性研究分析双侧入路经皮激光椎间盘减压术(Percutaneous Laser Disc Decompression,PLDD)联合穿通式臭氧消融术(Ozone ablation)治疗腰椎间盘突出症的长期疗效。方法回顾性分析2005年8月至2012年12月,采用PLDD治疗腰椎间盘突出症患者124例的临床资料。采用双侧入路PLDD联合穿通式臭氧治疗,术后3个月和5年随访,记录病变间盘突出物矢状径(Sagittal diameter of intervertebral disc herniation,H)与椎管矢状径(Sagittal diameter of vertebral canal,V)比值H/V,观察临床疗效和不良反应情况。结果 124例患者术后均未出现手术区神经损害及相应血管、腹部脏器损伤以及手术节段椎间隙感染等并发症。治疗前后H/V值比较,差异均具有统计学意义(P<0.05)。X线片测量显示无腰椎失稳或滑脱表现,脊柱力学稳定性均未受影响。依据修改后的MacNab腰腿痛手术标准进行评价,术后优良率分别为:3个月88.7%、5年87.1%(P>0.05),表明双侧入路PLDD联合穿通式臭氧治疗长期疗效稳定。结论双侧入路PLDD联合穿通式臭氧治疗腰椎间盘突出症长期疗效确切、稳定。
Objective To analyze the long-term efficacy of joint application of percutaneous laser disc decompression(PLDD)and ozone ablation in treating lumbar disc herniation through bilateral approaches.Methods A retrospective study was conducted on 124 patients with lumbar disc herniation treated with PLDD from Aug.2005 to Dec.2012.Bilateral approach PLDD plus penetrating ozone therapy was used.The ratio of sagittal diameter of intervertebral disc herniation(H)to sagittal diameter of vertebral canal(V)was observed 3 months and 5 years after the operation respectively.The clinical effect was observed and the adverse reactions were recorded.Results Complications such as nerve damage in operation area,corresponding blood vessel injury,abdominal organ injury and segmental intervertebral space infection did not occur in the 124 patients after the operation.The difference of H/V value before and after the treatment was statistically significant(P<0.05).X-ray measurements showed no sign of lumbar instability or spondylolisthesis,and spinal mechanical stability was not affected.According to the modified MacNab criteria for lumbago and leg pain surgery,the excellent and good rate was 88.7%in 3 months and 87.1%in 5 years after the operation(P<0.05),which indicated that the long-term efficacy of this method was stable.Conclusions The combination of bilateral approach PLDD and penetrating ozone is effective and stable in the treatment of lumbar disc herniation.
