摘要
奥沙利铂(草酸铂)属于第3代铂类制剂,抗癌谱广;其已在多种肿瘤中得到广泛应用,并被视为肠癌的一线用药。然而奥沙利铂的剂量限制性周围神经毒性目前尚无标准的防治方法,这影响其进一步的临床应用。方灿途教授认为奥沙利铂所致周围神经毒性的中医病机与脾虚、血瘀、寒滞有关,以气滞络瘀为特点,其治疗当以分清虚实缓急,以益气养血、活血通络、温经止痛为治则;自拟双筋龙汤(补阳还五汤的基础上去赤芍加宽筋藤、蜈蚣、桑枝、鸡血藤、桂枝、炙甘草而成)辨治奥沙利铂所致周围神经毒性,可改善病患化疗后四肢麻木的症状,提高患者的生活质量。
Oxaliplatin(Platinum Oxalate)is a third-generation platinum preparation with wide anti-cancer spectrum.It is widely used in various kinds of tumors and is used as the first-line drug for colon cancer.However,its dose-limiting peripheral neurotoxicity restricts its expansive clinical application,and currently there is no standard prevention and treatment method.Professor FANG Can-Tu believes that the pathogenesis of oxaliplatin-induced peripheral neurotoxicity is related with spleen deficiency,blood stasis,and cold stagnation,characterized by Qi stagnation and collateral blood-stasis;the treatment of oxaliplatin-induced peripheral neurotoxicity with traditional Chinese medicine should be on the basis of differentiation of deficiency or excess syndrome type,and chronic or acute stage.And the therapeutic principles are replenishing Qi and nourishing blood,activating blood to unblock collaterals,and warming meridians to alleviate pain.Professor FANG Can-Tu prescribes Shuangjinlong Decoction for oxaliplatin-induced peripheral neurotoxicity,a recipe mainly composed of Buyang Huanwu Decoction(with Radix Paeoniae Rubra removed)and together with Caulis Tinosporae Sinensis,Scolopendra,Ramulus Mori,Caulis Spatholobi,Ramulus Cinnamomi,and Radix Glycirrhizae Preparata,which can relieve the numbness of the four limbs and increase the quality of life of the patients.
