摘要
目的探讨甲状腺激素检测在慢性心功能不全(Chronic Heate Failure,CHF)患者中的临床意义。方法选择2017年2月—2018年3月CHF患者110例,根据NYHA心功能分级标准分为Ⅰ级组23例,Ⅱ级组27例,Ⅲ级组32例,Ⅳ级组28例,检测患者甲状腺激素TT_3、TT_4、FT_3、FT_4、TSH、TPO-Ab水平,并分析与心功能的关系。结果TT_3、FT_3、TT_4、FT_4、TSH及TPO-Ab水平在心功能Ⅰ级组分别为(1.7±0.2)ng/mL,(7.5±1.6) pg/mL,(128.4±11.6)μg/mL,(11.3±1.6)ng/mL,(4.1±0.2)IU/L及(22.5±4.1)IU/mL,在心功能Ⅱ级组分别为(1.6±0.2)ng/mL,(6.0±1.5) pg/mL,(127.4±10.6)μg/mL,(11.2±1.2)ng/mL,(4.1±0.3)IU/L及(22.6±3.6)IU/mL,在心功能Ⅲ级组分别为(1.3±0.3)ng/mL,(4.5±1.4)pg/mL,(129.45±10.4)μg/mL,(11.2±1.3)ng/mL,(4.0±0.2)IU/L及(22.4±4.4)IU/mL,在心功能Ⅳ级组分别为(1.0±0.3)ng/mL,(3.1±1.1)pg/mL,(127.4±11.7)μg/mL,(11.1±1.9)ng/mL,(3.5±0.5)IU/L及(22.5±4.8)IU/mL,心功能Ⅲ级组TT_3显著低于心功能Ⅰ、Ⅱ级组(P<0.05),心功能Ⅳ级组TT_3显著低于心功能Ⅲ级组(P<0.05);心功能Ⅱ级组FT_3显著低于心功能Ⅰ级组(P<0.05),心功能Ⅲ级组FT_3显著低于心功能Ⅰ、Ⅱ级组,心功能Ⅳ级组FT_3显著低于心功能Ⅰ、Ⅱ、Ⅲ级组(P<0.05);TT_4、FT_4、TPO-Ab各组比较差异无统计学意义(P>0.05);心功能Ⅳ级组TSH显著低于心功能Ⅰ、Ⅱ、Ⅲ级组(P<0.05)。结论慢性心功能不全患者TT_3、FT_3水平降低程度反映了心功能损害的严重程度,CHF患者血清甲状腺激素水平的改变对判断心功能及预后具有一定临床指导意义。
Objective To study the clinical significance of thyroid hormone test of patients with chronic cardiac insufficiency.Methods 110 cases of CHF patients admitted and treated in our hospital from February 2017 to March 2018 were selected and divided into four groups,according to the NYHA cardiac function classification standards,including 23 cases in levelⅠ,27 cases in levelⅡ,32 cases in levelⅢ,and 28 cases in levelⅣ,and the thyroid hormone TT3,TT4,FT3,FT4,TSH,TPO-Ab levels of patients were tested,and its correlation with cardiac function was analyzed.Results The TT3,FT3,TT4,FT4,TSH,TPO-Ab levels in the levelⅠ,levelⅡ,levelⅢ,and levelⅣof cardiac functions were respectively(1.7±0.2)ng/mL,(7.5±1.6)pg/mL,(128.4±11.6)μg/mL,(11.3±1.6)ng/mL,(4.1±0.2)IU/mL,(22.5±4.1)IU/mL,(1.6±0.2)ng/mL,(6.0±1.5)pg/mL,(127.4±10.6)μg/mL,(11.2±1.2)ng/mL,(4.1±0.3)IU/mL,(22.6±3.6))IU/mL,(1.3±0.3)ng/mL,(4.5±1.4)pg/mL,(129.45±10.4)μg/mL,(11.2±1.3)ng/mL,(4.0±0.2)IU/mL,(22.4±4.4))IU/mL and(1.0±0.3)ng/mL,(3.1±1.1)pg/mL,(127.4±11.7)μg/mL,(11.1±1.9)ng/mL,(3.5±0.5)IU/mL,(22.5±4.8)IU/mL,and the TT3 in the level III of cardiac function was obviously lower than that in the level I and level II of cardiac function(P<0.05),and the TT3 in the levelⅣof cardiac function was obviously lower than that in the levelⅢof cardiac function(P<0.05),and the FT3 in the levelⅡof cardiac function was obviously lower than that in the levelⅠof cardiac function(P<0.05),and the FT3 in the levelⅢwas obviously lower than that in the levelⅠand levelⅡ,and the FT3 in the levelⅣgroup was obviously lower than that in the levelⅠgroup,levelⅡgroup and levelⅢgroup(P<0.05),and the differences in the TT4,FT4,TPO-Ab between groups were not statistically significant(P>0.05),and the TSH in the levelⅣgroup was obviously lower than that in the levelⅠ,levelⅡand levelⅢgroups(P<0.05).Conclusion The decrease degree of TT3,FT3 levels of patients with chronic cardiac insufficiency can reflect the severity of cardiac function inj
