摘要
肾纤维化是肾实质逐渐瘢痕化,肾功能逐渐丧失的病理过程。作为各种肾脏疾病的后期改变,肾纤维化严重影响着患者的预后。因此,延缓及逆转肾纤维化的进展是慢性肾脏病(CKD)的重要治疗目标。腺苷是细胞外的一种小分子核苷,近年来发现CKD患者肾组织间隙腺苷浓度与纤维化程度相一致,提示腺苷信号通路在肾纤维化中发挥了重要的调节作用。细胞膜上主要有4种腺苷受体亚型,A1AR、A2AAR、A2BAR与A3AR。这4种腺苷受体在肾组织的分布以及相应的生物学效应有很大的差异,不同的腺苷受体可以促进或抑制肾纤维化的病变进展。并且,腺苷信号通路在肾组织内与其他信号通路相互联系,共同调控肾损伤修复以及CKD患者肾纤维化的进展。因此,腺苷信号通路可能成为治疗肾纤维化的重要靶点,但是其具体信号作用及下游通路仍有待更多的研究。此外,中医药对腺苷信号通路的调控作用也是中医药治疗CKD的重要作用机制,有待于进一步的探索与证实。
Renal fibrosis is a pathological process of gradual scarring of renal parenchyma and gradual loss of renal function. As a result of various renal diseases,renal fibrosis has a serious effect on the prognosis of patients. Therefore,delaying and reversing renal fibrosis is an important therapeutic target of chronic kidney disease(CKD). Adenosine is a kind of small molecules extracellular nucleotides,found that kidney tissues for patients with CKD in recent years gap adenosine levels consistent with the degree of fibrosis,prompt adenosine pathway plays an important regulating role in renal fibrosis. Adenosine receptors on the cell membrane,there are four main subtypes A1 AR,A2 AAR,A2 BAR and A3 AR,the four kinds of adenosine receptors in the renal tissue distribution and the corresponding biological effects are quite different,different adenosine receptors can promote or inhibit the pathological changes of renal fibrosis. Furthermore,adenosine signaling pathways are interrelated with other signaling pathways in renal tissue to jointly regulate renal injury repair and progression of renal fibrosis in CKD patients. Therefore,adenosine signaling pathway may be an important target for the treatment of renal fibrosis,but its specific signaling effect and downstream pathways still need to be studied. In addition,the regulation of adenosine signaling pathway by traditional Chinese medicine is also an important mechanism for the treatment of CKD,which need to be further explored and confirmed.
